- Description
- Provide primary care services to eligible veterans assigned to the contractor by the South Texas Veterans Health Care System (STVHCS), 7400 Merton Minter Blvd., San Antonio, TX 78229. NAICS Code is 621999; Size Standard is $8.5 million. This procurement is for full and open competition (Unrestricted). This procurement is being conducted IAW FAR Part 12, and Part 15. Contract will be based on an all inclusive capitated rate. Initial primary care visits will consist of physician's exam, clinical assessment diagnosis and treatment. Services are to be provided at the contractor's site. Specialty consultation and more extensive diagnostic workups (e.g., Cardiology, Pulmonary, Dermatology, and Computerized Axial Tomography) shall be referred to the STVHCS. The contractor must be able to supply laboratory and radiology services. The clinic(s) are required to be geographically located within/near the areas they are placing proposals. Service areas will be New Braunfels (Comal County), San Antonio (Bexar County), San Marcos (Hays County) and Seguin (Guadulape County), plus the surrounding counties of Atascosa, Blanco, Gonzales, Kendall, Medina, and Wilson. The contract period is for one year from date of award with two (2) additional one-year options. The government intends to make a single award, but reserves the right to make multiple awards. The solicitation will be posted on or about September 25, 2003. Offers will be due October 31, 2003, 5PM CST. No hard copies will be distributed. Any amendments issued to this solicitation will also be available at this URL address. Offerors are advised that they are responsible for obtaining amendments. Signing up for the bidder's list in FedBizOpps will automatically electronically provide you with any amendment notification to this solicitation. No fax or web-based proposals will be accepted. Solicitation Number RFP 671-12-04 shall be listed on the outside of offeror's proposal envelope for identification purposes.
