Description

NA (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals ar e being requested and a written solicitation will not be issued. (ii) This request for quotation (RFQ) number is DABQ03-04-T-0003. (iii) This RFQ and the provisions and clauses incorporated are those in effect through Federal Acquisition Circular (FAC) 200 1-15. (iv) This acquisition is 100 percent small business set aside, in regards to size standards 1,000 employees. The applicable NAICS is 325188 (v) This RFQ contains one (1) line item, Sodium Hypochlorite, QTY 500 DR., 12.5 percent solution strength (53 gallon drum) an MSDS must accompany each delivery. STATEMENT OF WORK FOR SODIUM HYPOCHLORITE (53 GALLON DRUM) Contractor shall provide all transportation and other necessary materials to supply this requirement for Sodium Hypochlorite (12.5% solution stre ngth) in 53-gallon drum to the Water Treatment Plant, Bldg 3565, Fort Wainwright, AK. The contractor shall also pick up empty drums from the same Bldg location when full drums are delivered. A) Approximate consumption per day is 40 ?????? 60 gallons. Fo r the period of this contract, consumption will be approximately 15,900 gallons minimum and 26,500 gallons maximum. B) The contractor will be notified telephonically when eight (8) empty drums are on hand at the Water Plant, Bldg 3565. Upon notification, the contractor shall deliver within four (4) calendar days remove the eight (8) empty drums and provide eight (8) full drum. C) All pick up and delivery under this contract will occur between 0800 and 1400 hours Monday thru Friday, except Holidays. D) Per sons authorized to place calls under this contract will be provided in the award. E) Upon initial delivery of the first time (8) drums, the Water Plant will provide eight (8) empty sodium hypochlorite drums and eight (8) empty drums for each delivery ther eafter. F) Since empty drums will be exchanged for full drums, it is requested that this contract be offered without a deposit charge for drums. (vi) The delivery date will be called in after award. Delivery location, Acceptance will be Government FOB dest ination to Fort Wainwright, Alaska 99703. (viii) The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition, along with the following addenda. (ix) Offers will be evaluated using the following evaluation factors technica l acceptable, price and delivery time. The contract will be awarded to the offeror that provides the best value to the government. (x) All offers must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Comm ercial Items (Alt I) (See attached) (xi) The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items applies to this acquisition along with the following addenda: 52.219-1, Small Business Program Representations (Apr 2002).
