Description

NA (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals ar e being requested and a written solicitation will not be issued. (ii) This request for quotation (RFQ) number is W912CZ-04-T-0003. (iii) This RFQ and the provisions and clauses incorporated are those in effect through Federal Acquisition Circular (FAC) 200 1-15. (iv) This acquisition is 100 percent small business set aside, in regards to size standards 1,000 employees. The applicable NAICS is 325188 (v) This RFQ contains one (1) line item, Hydrogen Peroxide, FFP ?????? Annual contract to provide 50% hydrogen peroxide solution, 8000 gallon minimum to 10000 gallon maximum per year. Initial delivery to be made in 300 gallon containers; perhaps to be converted to bulk delivery during the course of the contract terms from 20 Oct 2003 through 30 Sep 2004. QTY EST . 33 Containers, an MSDS must accompany each delivery. All pricing has to be destination Fort Wainwright, AK 99703. (vi) All delivery under this contract will occur between 0800 and 1400 hours Monday thru Friday, except Holidays. Person(s) authorized to place calls under this contract will be provided in the award. (vii) Acceptance will be Government FOB, Power Plant, Bldg 3595, Fort Wainwright, Alaska 99703. (viii) The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquis ition, along with the following addenda. (ix) Offers will be evaluated using the following evaluation factors technical acceptable, price and delivery time. The contract will be awarded to the offeror that provides the best value to the government. (x) All offers must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items (Alt I) (See attached) (xi) The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items applies to this acquisi tion along with the following addenda: 52.219-1, Small Business Program Representations (Apr 2002).
