- Q- Delivery and Installation of DME equipment: VISN 9 is submitting a request for providing delivery and set up of durable medical equipment for the medical centers in VA TN Valley (Nashville and Murfreesboro, TN), Memphis and Mountain Home, TN; Lexington and Louisville, KY; and Huntington WV. The period of service is one base year with four one year option periods. All responsible sources may submit a proposal which shall be considered by the agency. Only written requests will be accepted. You may fax your request to 6l5 867-5429. All questions must be written and may be faxed to the same number. This is a l00% small business set aside in accordance with NAICS code of 621610
