Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation F73MED326131 is issued as a request for quotes. This solicitation document & incorporated provisions & clauses are those in effect through Federal Acquisition Circular 2001-15. This procurement is a 100% small business set-aside. The NIACS Code is 621512 with a size standard of $8,500,000. You must be registered in the Central Contractor Registration (CCR) to bid on this solicitation. The contractor will invoice on a monthly basis for the work performed. DESCRIPTION: LICENSES/APPROVAL: 1.1 The Radiology Department of the 97th Medical Group will be the only point of contact for arranging interpretation of developed x-ray film for the 97th Medical Group. 1.2 The Contractor shall provide evidence that employees reviewing the developed x-ray film are board certified, or board eligible, by the American Board of Radiology and shall possess an unrestricted medical license to practice radiology from a U.S. jurisdiction. 1.3 The contractor shall submit their detailed quality assurance program to the Government prior to start of services. PERIOD OF PERFORMANCE: The estimated period of performance is from 20 Oct 03 through 30 Sep 04. STATEMENT OF WORK: See attached document. The following clauses and provisions are incorporated and are to remain in full force in any resultant purchase order: FAR Clause 52.212-1, Instructions to Offerors- Commercial Items (Oct 2000). It is amended to read: Submit signed and dated offer to 97 CONS/LGCA, 303 J Ave Bldg 302, Altus AFB OK 73523-5147 at or before 1600 (4:00 P.M) CST, 10 Oct 2003. Submit dated quotations on company letterhead or quotation form. Quotations may be faxed to (580) 481-5138. Each offeror shall include a completed copy of the provisions at FAR clause 52.212-3, Offeror Representations and Certifications?Commercial Items (June 2003) with the quotation. If a copy of the provisions are needed, the can be found on arnet.gov/far or as an attachment to this solicitation. FAR 52.212-2 also applies to this solicitation and any resultant contract. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Price and technical capability of the item offered to meet the Government requirement. It is incorporated by reference and applies to this acquisition.
