- Description
- This is a combined synopsis/solicitation for commercial service prepared in accordance with the format in FAR subpart 12.6 ? Streamlined Procedures for evaluation and solicitation for commercial items as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This solicitation is a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-15. The associated North American Industrial Classification System (NAICS) code for this procurement is 812930. The Food and Drug Administration (FDA) has a requirement for 20 regular parking spaces. The parking space must be within reasonable walking distance of 409 3RD Street SW. The following boundaries will be used in determining the final parking lot for the required spaces. The blocks of Virginia Avenue, 4th Street and C St. SW. Period of performance is for twelve months from date of award. An award will be made fifteen (15) days from date of this notice. The following provision & clauses apply to this acquisition: 52.212-1, Instructions to Offerors-Commercial Items; and 52.212-3 Offeror Representations and Certifications-Commercial Items. The following clauses apply to this acquisition: 52.212-4, Contract Terms and Conditions-Commercial Items; 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items. All offerors must submit the following: 1) A quotation; 2) product literature; 3) A copy of the most recent published price list(s); and 5) a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items which may be downloaded at www.arnet.gove/far. The FDA strongly encourage all vendors to register with the Central Contractor Registration (CCR) System. By October 1, 2003, all vendors that do business with the FDA and the Federal Government will be expected to have registered in CCR. You must have a DUNS number to begin your registration. Call Dun & Bradstreet, Inc. at 866-705-5711 and the CCR website is www.ccr.gov. All quotes shall be sent to The Food and Drug Administration, Office of Acquisitions,Grants and Central Services, Attn: Dee Gibson, Room 2131, 5600 Fishers Lane, Rockville, MD 20857. Submission must be received by 5:00p.m. local time on October 17, 2003. Fax and e-mail quotes will be accepted.
