- Description
- The Louis Stokes Cleveland (LSC) Department of Veterans Affairs Medical Center (DVAMC) has a requirement for Commercial Services of a prospective contractor to Install (Lay) Government-Provided-Cabling at facilities under its cognizance, as cited below. The Solicitation shall be issued as a Request For Quote (RFQ), Numbered 541-R-005-04 and is being prepared in accordance with a combination of applicable Federal Acquisition Regulation (FAR) Parts, 12, 13, 15, and 19. This is a 100% Set-Aside for Small Business Concerns within a fifty ?(50) mile radius of Cuyahoga County, Ohio. The North American Industry Classification System (NAICS) Code is 237130, entitled Cable Laying (e.g., cable television, electricity, marine, telephone), including underground. The Small Business Size Standard is $28.5 million. The selected contractor shall be required to provide all labor and peripheral materials as required to provide services, entitled Installation and/or Laying of Government Provided Cabling at any of its facilities under the cognizance of the LSC DVAMC, which encompasses the Brecksville Facility, located at 10000 Brecksville Road, Brecksville, Ohio 44141; the Wade Park Facility, located at 10701 East Boulevard, Cleveland, Ohio 44106; and Community Based Outpatient Clinics, and Field Service Centers throughout Northeast Ohio. The work shall be in compliance with standard commercial procedures and in accordance with the Statement of Work. The period of performance for the Base Year is November 01, 2003 through September 30, 2004 with two (2) one-(1) year pre-priced option renewals. The RFQ solicitation document will be prepared in MS Word Format and will be available by download only from the Government Point of Entry Internet Site, entitled Federal Business Opportunities for Vendors at http://www.eps.gov. Select VA (for Veterans Affairs) and the location is VAMC Brecksville (Cleveland). NO HARD COPIES OF THE RFP SOLICITATION WILL BE MAILED. The RFP will be available on or about October 15, 2003 and proposals will be due at the Issuing Office on or about October 29, 2003 at the Hour of 4:00 PM, Eastern Standard Time (EST). Technical Questions in the form of a Request For Information (RFI) shall be submitted in writing and/or may be mailed to LSC DVAMC, A&MMS, Building 1, Room A-104, 10000 Brecksville Road, Brecksville, Ohio 44141 (Attn: Lloyd BRODNAX) ?or-transmitted via facsimile number 440.838-6052. Note: The cut-off date for RFI?s is October 22, 2003. Any Government response(s) to an RFI will be published via an amendment to the RFP Solicitation using Standard Form (SF) 30, entitled Amendment of Solicitation/Modification of Contract and posted to the www.eps.gov Internet Site. Numbered Notes 1 and 13 are applicable to this acquisition.
