Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation FA4620-Q-A003 is issued as a request for quote, utilizing Simplified Acquisition Procedures in FAR Part 13. A firm-fixed price type contract will result from this solicitation. The performance period will be for an 11-month base year with an anticipated start date of 01 November 2003, and 4 one-year option periods beginning 01 Oct 2004 ? 30 Sept 2008. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-15. The applicable NAICS Code is 811310. Small business size standard is $6.0 M. This is a 100% small business set-aside. The major elements of service include the maintenance, inspection, repair and preventative maintenance of hoists, cranes and vehicle lifts at different locations throughout Fairchild AFB, in accordance with the attached statement of work. The following attachments are incorporated as part of this solicitation and can be downloaded at link: http://www.eps.gov . 1) Pricing Schedule, 2) Statement of Work, and 3) Wage Determination No. 94-2565 Rev (19) Area: WA, Spokane. The provisions at FAR 52.252-1 Provisions Incorporated by Reference (Feb 1998) [the fill-in is completed with: http://www.arnet.gov/far and http://farsite.hill.af.mil], and FAR 52.212-1: Instructions to Offerors ? Commercial Items (July 2003) apply to this acquisition. Addendum to para (b) (10) is tailored as follows; Past Performance Information. Provide references for same or similar type contracts completed in the past three (3) years. Provide any information currently available (letters, metrics, customer surveys, etc.) which demonstrates customer satisfaction with overall job performance and quality. Furnish the following information for each reference: (i) Company/division name; (ii) Point of Contact; (iii) Telephone numbers/Fax; (iv) Description of Service; (v) Contract Number; (vi) Contract Dollar value]. FAR 52.212-2 Evaluation?Commercial Items (Jan 1999) applies to this acquisition. Addendum: this clause is tailored as follows: (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (i) Price; (ii) Past Performance; (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s); (c) Using questionnaires, the government will seek performance information based on the offeror?s references and data independently obtained from other government and commercial sources. Past Performance regarding predecessor companies, key personnel who have relevant experience or subcontractors that will perform major or critical aspects of the requirement will not be weighed as highly as past performance information for the principal offeror. The Contracting Officer shall make an integrated assessment considering past performance and price to select the offer with the best overall value to the government; (d) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are discussions after its receipt, unless a written notice of withdrawal is received before award.
