- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, streamlined procedures for evaluation and solicitation for commercial items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This solicitation, FA3022-04-T-0001 is a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-16. The associated North American Industry Classification System (NAICS) for this procurement is, 423450 and the small business size is 100 Employees. This is advertised as 100% Small Business Set-Aside. Contractor to provide 1 - TOPCON NW6S Digital Retinal Camera with TV Relay lens. Model #TRC-NW6S. Includes, PC Mini-Tower with Imagnet Light software, Nikon D100 Digital Camera/6.1 Pixel and photo quality printer. 1 ? TOPCON AIT-15 Table W/O Top and 1 - TOP-LAR, Large top for table, Size 18?x33? or equal. If equal model is quoted please provide detailed specification with quote. FOB: Destination of delivery will be to 14th Medical Squadron, 201 Independence, Suite 101, Columbus AFB, MS 39710-5300. All interested parties must be registered with the Central Contractor Registration (CCR) as prescribed in DFARS 252.204-7004 and 252.232-7009. CCR can be accessed at www.ccr.gov or by calling 1-888-227-2423. The CCR registration must be completed before award can be made. All offers should be faxed to 2Lt Romo-Garza at 662-434-3049 or emailed to david.romogarza@columbus.af.mil. The due date for all quotes is 4 November 2003 by 4:00 p.m. CDT. Note that addendum to 52.212-1, Instructions to Offerors-Commercial Items page of this solicitation, applies to this acquisition. Offerors shall complete and present with their offers FAR 52.212-3 Offeror Representations and Certifications - Commercial Items (June 2003).
