- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. (i) The solicitation number is FA4488-04-T-0002 and the solicitation is issued as a request for quotation (RFQ). (ii) This solicitation document incorporates provisions and clauses in effect through Federal Acquisition Circular 2001-16. (iii) This solicitation is 100% set-aside for small business concerns. The associated NAICS code is 611620 and small business size standard is $6.0 million. (iv) This requirement consists of one line item to be priced on a monthly basis for 11 months. This requirement also contains two option years, to be priced on a monthly basis for 12 months each respectively. (v) This requirement is for Aerobic Instruction Services. The Contractor shall provide qualified aerobic instructors in accordance with the Statement of Work. Qualified aerobic instructors shall hold certificates from nationally recognized institutions: American Council on Exercise (ACE), American and Fitness Association of America (AFAA), and American College of Sports Medicine (ACSM). All proposals shall include this documentation of qualification. (vi) The expected period of performance of any resulting contract will be from 3 Nov 03 through 30 Sep 04. The two option years? performance shall be from 1 Oct 04 through 30 Sep 05, and 1 Oct 05 through 30 Sep 06. When submitting a quote be sure to include pricing per month and per year for each performance period (one base year and two option years) in addition to the total price for the entire contract. (vii) The provision at FAR 52.212-1, Instructions to Offerors -- Commercial, applies to this solicitation. (viii) The provision at FAR 52.212-2, Evaluation -- Commercial Items, applies to this solicitation. Paragraph (a) of FAR 52.212-2 is added to read: ?Best Value?. The contractor certifies that it can meet all the requirements by submitting a quotation. (ix) The provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items, applies to this solicitation. The contractor shall return a completed copy of this provision with its quotation. A copy of the provision may be attained from http://farsite.hill.af.mil. (x) The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. Paragraph (i) is changed to read: ?Payment will be made on a monthly basis?. The government shall only be liable to the contractor for performance up to the terminate date in the notice. (xi) The clause at FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition. The following FAR clauses identified at paragraph (b) of FAR 52.212-5 is considered checked and is applicable to this acquisition: (13), (14), (15), (16), (17), (18), (19), (24), and (29). The following FAR clauses identified at paragraph (c) of FAR 52.212-5 are considered checked and are applicable to this acquisition: (1) and (2). (xii) The clause at FAR 52.217-7, Option for Increased Quantity?Separately Priced Line Item, applies to this acquisition. FAR 52.217-7 is added to read: ?30 calendar days?. (xiii) The clause at FAR 52.217-9, Option to Extend the Term of the Contract, applies to this acquisition. Paragraph (a) of FAR 52.217-9 is added to read: ?30 calendar days? and ?60 calendar days? respectively. Paragraph (c) of FAR 52.217-9 is added to read: ?35 months/2 years 11 months?. (xiv) The clause at FAR 52.232-33, Payment by Electronic Funds Transfer?Central Contractor Registration, applies to this acquisition.
