(Dosimetry Treatment Planning Computer) USV-31401FN PIN: 00183-92085 Location Radiation Oncology DepartmentMILSTRIP: N0018304RQRT010PURCHASE REQUEST NUMBER: N0018304RQRT010 NET AMT FOB: Destination Section C - Descriptions and Specifications CLAUSES INCORPORATED BY FULL TEXT STATEMENT OF WORK Contractor to provide maintenance support for the Hewlett Packard Focus Treatment Planning System (location Radiation Oncology) in accordance with the original equipment manufacturer?s (OEM) recommendations. Contractor shall provide maintenance service that includes on-site repairs/support during normal working hours of 7:00 a.m. to 6:00 p.m., Monday through Friday excluding federal holidays. Maintenance support includes all systems, subsystem components, and assemblies that are part of the original system purchased. All maintenance provisions shall apply to hardware, firmware, and software, as appropriate, unless otherwise stated. 1. Service to be provided as needed to maintain equipment in good operating condition. Maintenance may be performed concurrent with remedial repair. 2. Contractor shall perform any necessary remedial maintenance and repair. The Contractor shall start any emergency repairs required to restore equipment to the level required for operation of its primary software within sixteen (16) business hours after notification by Government or request for service. 3. Maintenance support includes all labor, travel, and parts (excluding consumable supplies and accessories) determined necessary by Contractor to maintain equipment in good operating condition. Replacement parts shall be new or remanufactured (with like new warranty). Contractor installed replacement parts shall become property of the Government and replaced malfunction parts shall become property of the Contractor. 4. The Government will be responsible for maintaining the proper environment, including utilities and site requirements necessary for the system to function properly as specified by the OEM. The Government will operate the system in accordance with the instruction manual provided by the OEM. 5. FOCUS/XIO Radiation Treatment Planning Hardware consist of the following: a. High Resolution Workstation 3 ea b. Resolution Videographics Display Unit 3 ea. c. Spaceball tm Graphics Control Devices 3 ea. d. DAT Tape Drive 1 ea. e. Printer / Plotter 1 ea. f. Accugrid Digitizing Tablet 1 ea. g. Modem 1 ea. h. Ethernet tm Transceiver 3 ea. i. UNIX and /or LINUX Operating System 3 ea. Vendor to cover all hardware purchased from CMS for use with FOCUS/X10 Radiation therapy treatment Planning System. (vendor not required to maintain ?pass-through? items)
