Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The reference number for this request for quotation is F13MED32930100. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 01-17 and Defense DCN 20031001. If two (2) or more responsive, responsible and technically acceptable small or HUBZone businesses respond, this acquisition will be 100% set-aside for small business, if not, it will be unrestricted. The Federal Supply Class (FSC) is W065. The associated Standard Industrial Classification (SIC) is 7352. The North American Industry Classification System is 532490 and the associated business size standard is $6 Million. This request for quotation consists of the following: Services Nonpersonal: To provide all labor, tools, parts, materials, facilities and transportation necessary to rent a GE helical mobile CT scanner to be used in interim of installation of new CT scanner. Rental Period: 24 Nov 03 through 24 Dec 03. Scanner Specs: GE HiSpeed helical CT Scanner with highlight detector & slip ring technology. 48x8' Calumet Trailer (with 6' pop out), patient lift, lighting, leveling system, phone connection & fresh/waste holding tanks. Kodak Dryview laser camera. Helical scan times 0.6, 1, 2, 3, 4 second with cine option. Optical disk archiving. Operator?s workstation, Plasma touch screen and sink. Mobile CT requires 480V/2 Phase/100A (Min) shore power. Dicom 3.0 compatible; full network capabilities. Cost of rental for 1 month_________________.
