- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; a quote is being requested and a written solicitation will not be issued. The NAIC code is 811490 with a size standard of 100 employees. This request for quotation (RFQ) number 04DWEL000045 is for Nustep and Cybex Fitness Equipment to equip a new facility and initiate a comprehensive wellness/fitness program for Federal Occupational Health. The delivery date shall be December 1st, including full setup and installation of the equipment. Delivery shall be to EPA Fitness Center, 1310 L Street, Room 140, Washington, DC 20005. The equipment shall include the following items WITH NO SUBSTITUTIONS: 1) Nustep TSR 4000 Seated Stepper, Quantity 1, 2) Cybex VR 2 Dual Axis Chest Press, Item Number 4507, Quantity 1, 3) Cybex Lateral Pulldown, Item Number 4516, Quantity 1, 4) Cybex Overhead Press, Item Number 4526, Quantity 1, 5) Cybex Row/Rear Deltoid, Item Number 4520, Quantity 1, 6) Cybex Leg Extension, Item Number 4612, Quantity 1, 7) Cybex Leg Curl, Item Number 4627, Quantity 1, 8) Cybex Seated Leg Press, Item Number 4605, Quantity, 1, 9) Cybex Hip Adduction, Item Number 4640, Quantity 1, 10) Cybex Cable Column with Booms, Item Number 5316, Quantity 1, 11) Cybex Twin Tier Dumbell Rack, Item Number, 5380 12) Set of Dumbells, 5-50 lbs, 10 pieces, Quantity 1. The quotation shall provide a firm-fixed unit price, inclusive of all charges. Award will be made on an all or none basis in the form of a purchase order on a fixed-price basis to the offeror providing the lowest cost to the Government. The Contractor shall include a fully executed copy of the Federal Acquisition Regulation (FAR) provision 52.212-3, Offeror Representations and Certifications-Commercial Items (JUN 2003). The provisions and clauses applicable to and incorporated in this RFP are those in effect through Federal Acquisition Circular 17 (OCT 2003).
