Description
The Centers for Disease Control and Prevention (CDC), National Center for Chronic Disease Prevention and Health Promotion (NCCDPHP), Division of Adolescent and School Health (DASH), contemplates awarding a contract to develop and conduct in the year 2006 the third School Health Policies and Programs Study (SHPPS 2006). The study will help a) characterize all eight components of the school health program in grades K-12 nationwide in elementary, middle, and senior high schools; and b) identify the extent to which school policies and programs address six categories of behaviors related to the leading causes of mortality, morbidity, and social problems among youth and adults in the United States such as: 1) behaviors that contribute to unintentional injury and violence; 2) tobacco use; 3) alcohol and other drug use; 4) sexual behaviors that contribute to unintended pregnancy and sexually transmitted diseases, including HIV infection; 5) dietary behaviors; and 6) physical inactivity. The study also will provide data to help monitor relevant National Health Promotion and Disease Prevention Objectives and other critical school health issues. Award of a Cost-Plus-Fixed-Fee contract in accordance with FAR Part 15 is contemplated. The anticipated period of performance is as follows: Phase I - April 2004 to April 2005; Phase II - May 2005 to December 2005; and Phase III - January 2006 to December 2006. CDC will consider all responsible sources responding to this Request for Proposals (RFP). All questions/concerns about the RFP shall be submitted via e-mail to BBiltz@cdc.gov. The tentative issue date is November 14, 2003, with the RFP issue date approximately 30 days after posting of the RFP. The complete solicitation package will be available and posted at this site for downloading when issued. No telephone requests of the solicitation will be accepted. No hard copy of the solicitation package will be mailed. The associated NAICS codes are 541710 or 541690 and the small business size standard is $6 million.
