Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the Federal Acquisition Regulation (FAR) sub part 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation and THERE WILL BE NO WRITTEN REQUEST FOR QUOTATION ISSUED. The solicitation number for this Request for Quotation is RFQ-797-FDF8-04-0007. This solicitation is 100% set aside for Small Business. This solicitation is issued on a restricted basis as a total small business set aside under NAICS Number 339112. DESCRIPTION: 5 EACH 5mm 30 DEGREE FULL SCREEN AUTOCLAVABLE LAPROSCOPE, 3 EACH 0 DEGREE 10MM AUTOCLAVABLE LAPROSCOPE, 1 EACH 45 DEGREE 10 MM AUTOCLAVABLE LAPROSCOPE, 4 EACH 6.5 MM 10 FT CLEAR CASE FIBER OPTIC LIGHT CABLE, 2 EACH 4.0 MM 30 DEGREE C-MOUNT SCOPE, 1 EACH 4.0 MM 70 DEGREE C-MOUNT SCOPE, 4 EACH C-MOUNT CAMERA HEAD ASSEMBLE 10? CABLE WITH COUPLER. Delivery, which shall be FOB Destination to NELLIS AFB NV 89191 and shall occur 30 days after award of the purchase order. Offerors responding to this announcement shall provide all information contained in the Federal Acquisition Regulation (FAR) Clause 52.212-1, Instructions to Offeror-Commercial Items. As stated in this clause, the Government intends to evaluate offers and award a contract without discussions with offerors. Therefore, the offeror?s initial offer should contain the offeror?s best terms from a price and technical standpoint. However, the Government reserves the right to conduct discussion if later determined by the Contracting Officer to be necessary. Offerors shall include a completed copy of provision 52.212-3, Offeror Representations and Certification--Commercial Items. An authorized representative of the offeror must sign these representations and certifications; (4) The Offeror shall also provide its Commercial and Government Entity (CAGE) code, Contractor Establishment Code (DUNS number) and Tax Identification number. The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-4, Contract Terms and Conditions--Commercial Items. Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items, applies to this acquisition.
