Description
The proposed contract action is for supplies or services for which the Naval Medical Center San Diego intends to solicit with only one source under the authority of FAR 6.302. Interested contractors/persons may identify their interest and capability to respond to the requirement or submit quotations. This notice of intent is not a request for competitive quotations. However, all quotes received November 07, 2003 @ 10:00 a.m. Pacific Standard time, will be considered by the Government. A determination by the Government not to compete with this proposed contract based upon responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes only solicitation, quotations are being requested and a written solicitation will not be issued. The solicitation number, N00259-04-T-0004 is issued as a request for quotation (RFQ). 0001. Job Service - Non-Personal: Indefinite Type Contract: Contractor shall provide supervision, labor, material, transportation and parts to Provide an Repair/Replacement Plan for Linvatec Rigid Endoscopes, excluding small joint (2.4mm) scope, located at Naval Medical Center San Diego.Contractor shall perform in accordance with the provided statement of work. Contractor performance period: November 10, 2003 thru September 30, 2004 0001AA Endoscopes 60 Each @ $____ = $____ Repair/Replacement Plan, Name: Rigid Endoscopes, Size: 1.9MM, 2.4MM, 2.7MM And 4.MM Statement of Work Repair Service & Support for Linvatec Rigid Endoscope Naval Medical Center San Diego, as customer agrees to send all of its Linvatec rigid endoscopes, excluding small joint (2.4mm) scopes, to Linvatec for repair and Linvatec agrees to send to Customer replacement Linvatec endoscope for a fixed fee. If Linvatec provides a replacement endoscope within two (2)business days of being notified by Customer that a Linvatec endoscope needs repair, Customer will pay the fee stated below. If the replacement scope is not received by Naval Medical Center San Diego within two (2) business days after notification, the government will not be charged, provided, however, that the damaged endoscope is returned to Linvatec and that delivery delays due to force majeure (e.g. natural disasters, strikes or other circuytstances beyond Linvatec control) will not excuse the government paying the fee. Insure that only FULLY QUALIFIED FIELD ENGINEERS and TECHNICIANS who have gone through original equipment manufacturer (OEM) or comparable third party service schools for the above mentioned equipment, shall be employed in the performance of any and all work performed under this contract. Upon request, the Medical Repair Branch will provide the certificates or notarized copies for verification. The highest standard of professional capability and electrical/mechanical workmanship is to be maintained throughout the life of this contract. Make repairs to the extent necessary (as determined by inspection tests or disassembly) to ensure a functional system that will efficiently serve its intended purpose. Perform service maintenance to industry standards. Provide only the work necessary to restore the equipment to a serviceable! operating condition by adjustments, replacement parts, or minor repairs when it is determined that extensive repairs and parts replacements are not necessary. Notify the Medical Repair Branch immediately upon receipt of OEM or replacement parts/equipment safety recalls notices. Insure that original design and functional capabilities will not be changed, modified, or altered unless the Medical Repair Branch authorizes such changes. Provide suitable modem recommended repair equipment! tools required for the satisfactory execution of all repairs made. Furnish manufacturer OEM approved lubricants and lubricate wear points within the equipment. Extend to the Government all commercial warranties or replacement parts, consistent with standard industry practices. We required a refurbish exchange program where NMCSD send the broken scope. We required to receive refiubish and re-condition scope with the same model and manufacturer items. Procedure 1. Naval Medical Center San Diego will notify Linvatec Customer Service (at 800-237-0169) when a scope is damaged and replacement is requested. 2. Linvatec Customer Service will issue a Return Goods (RG) number to the Customer, and instruct Customer to. return to return the damaged, endoscope to a designated Linvatec Authorized Repair Facility. 3. Naval Medical Center San Diego will, at its expense, send the damaged endoscope to the designated Repair Facility. 4. . Linvatec will, upon issuance of the RG number, invoice shall be in arrears although, upon receipt of the Government damaged endoscope, and verification that it is covered by this Agreement and not covered under Linvatec's warranty, Linvatec will credit the Government. 5. If the damaged endoscope is covered by Linvatec's warranty, Government will be credited. 6. A credit will not be issued to Government if the damaged endoscope is not received by the designated Repair Facility with in (30) days from the shipping date of the replacement endoscope. -End of Statement of Work- The solicitation document and incorporated provisions and clauses are those in effect through FAC 2001-15. Commercial Items (1999), the following factors shall be used to evaluate offers: 1) Past performance 2) Technical qualifications based on SOW 3) Price. Past performance and Technical qualifications based on SOW when combined it has more importance than cost or price. Accordingly, the contractor shall furnish such Technical qualifications as stated in the Statement of Work (SOW) to the Purchasing Agent along with their quote. All responsible sources may submit a quote which shall be considered.
