Description

This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. A quote is being requested and a written solicitation will not be issued. Solicitation number F73FAB32390100 is hereby issued as a Request for Quote. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-14. This acquisition is unrestricted under North American Industry Classification System code 561720. This requirement is for Removal of Dead Birds and Bird Waste from Four Hangars on Altus AFB OK. The following clauses and provisions are incorporated and are to remain in full force in any resultant contact: FAR 52.212-1, Instructions to Offers-Commercial Items(Oct 2003). It is incorporated by reference and applies to this acquisition. It is amended to read: Submit signed and dated offer to 97 CONS/LGCA, 303 J Ave, Bldg 302, Altus AFB OK, 73523-5032 at or before 1630 (4:30 P.M.) CST, 24 Nov 2003. Submit signed and dated quotations on company letter head or quotation form. Quotations may be faxed to (580) 481-7025. OFFERS WILL BE EVALUATED ON THE BASIS OF PAST PERFORMANCE AND BEST VALUE TO THE GOVERNMENT. In evaluation, Past Performance shall be approximately equal to price. Submit 3 references of Past Performance within the last 5 years with your bid. The Government reserves the right to make award on the initial quote without discussions. Each offeror shall include a completed copy of the provisions at FAR 52.212-3, Offeror Representations and Certifications/Commercial Items (Jun 2003) with the quotation. If a copy of the provisions are needed, please request one from the point of contact, and a copy will be faxed. FAR 52.212-4 (Feb 2003), Contract Terms and Conditions/Commercial Items, is incorporated by reference and applies to this acquisition with the following addendum: FAR 52.212-4 (c ) Changes. FAR 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders/Commercial Items (Jun 2003) and its deviations (May 2002) applies to this acquisition and is incorporated by reference. For Information Only: FAR 52,222-42 Statement of Equivalent Rates for Federal Hires (May 1989): Laborer, $8.56/hr, $2.52 Fringe Benefits. IAW FAR 52.237-1, site visit is scheduled for 14 Nov 03, 14:30 hrs (2:30 P.M.) CST. Meet on Altus AFB OK, Bldg 302, LGC Conference Room. Call POC when you arrive at the gate for escort into the base. BID schedule (See Atch 1), Statement of Work (See Atch 2), Wage Determination 1994-2525, Rev 23, (See Atch 3) all are hereby incorporated into this solicitation. A Firm-Fixed Price Contract, Basic plus 4 Options, will be issued in writing. No telephonic responses will be processed.
