- This is a combined synopsis/solicitation for commercial items and services prepared in accordance with the format in FAR subpart 13.5, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-15. This acquisition is a Small Business Set-Aside Acquisition limited to Small Businesses within a 100-mile radius of Altus Air Force Base, Oklahoma. The North American Industry Classification Standard (NAICS) code 238330. The following Statement of Work details the specifications. Award shall be made only to contractors who have registered with the Central Contractor Registration at: www.ccr.gov/. STATEMENT OF WORK FLOOR COVERING INSTALLATION 6 Nov 03 1.DESCRIPTION OF SERVICES: The contractor shall provide all personnel, equipment, tools, materials, supervision, administration and services necessary to remove existing floor covering materials and accessories (cove base, inside/outside corners) and install new Government furnished carpet, (broadlooms and carpet tile), contractor furnished cove base and accessories (to include inside/outside corners), at numerous buildings at Altus Air Force Base, Oklahoma. Contractor will provide own equipment to move the government furnished carpet/carpet tile from point of storage to point of installation. Contractors will submit a proposal based on removing existing floor covering materials and accessories and installing: 15,155 SY of carpet tile (govt furnished); 4,352 SY of Broadloom carpet (govt furnished); 1,011 inside/outside corners (contractor furnished); and 32,356 LF of cove base (contractor furnished). At a minimum, the contractor shall provide an on-site foreman who is currently certified by the International Certified Floor covering installers Association, Commercial II or higher. The foreman shall be on Altus AFB at all times while work is being performed. Contractor shall provide copies of certification to the Contracting Officer with their proposal. 1.1 COORDINATION OF WORK: The contractor shall coordinate with QAE before the removal and installation of any floor coverings. A copy of building drawings with areas marked to receive new carpet/carpet tile will be given to the contractor upon award of purchase order. Contractor is responsible to field verify exact measurements and quantities of the area(s) to receive installation. Contractor will provide a schedule based on estimated installation dates. Estimated start date will be 8 Dec 03. 1.2 REMOVAL OF FLOOR MATERIALS: The contractor shall remove existing flooring materials to accommodate the installation of new materials. 1.3 CLEANUP/DISPOSAL: Vacuum newly installed carpet using a machine with a face-beater bar element. Remove spots or stains and replace flooring materials where spots cannot be removed and clean base cove of all foreign materials. Clean all other flooring materials according to the manufacturer recommendations and specifications. The contractor is responsible for removing all waste materials and debris from the installation site and discarding at a state permitted disposal site. 1.4 FLOOR PREPARATION: Flooring shall be in a smooth, true level plane except where indicated as a sloped area. 2. FLOORING MATERIALS: The contractor shall use adhesives, seam sealers and seam tape recommended by manufacture. 2.1 RUBBER COVE BASE: Rubber cove base will be installed with factory recommended adhesives. Cove Base Material: Armstrong, Roppe or CO approved equal. Thickness:1/8? Height: 4? Length: 120? Coils Color and pattern to be selected by the government from the contractor?s showroom display and manufacture?s standard samples. Each building may have a different color rubber cove base. 2.2 INSIDE AND OUTSIDE CORNERS: Factory inside and outside corners will be installed with factory recommended adhesives. Material: Armstrong, Roppe or CO approved equal. Thickness: 1/8? Height: 4? Color and pattern to be selected by the government from the contractor?s showroom display and manufacture?s standard samples. All inside and outside corners will be factory made with the same height and thickness as straight runs of cove base being used. 2.3 WOODEN BASEBOARDS: There may be buildings or rooms with wooden baseboards that may have to be removed prior to installation of new carpet. Wooden baseboards shall remain and be reinstalled; the contractor shall replace damaged wooden baseboards, to match existing, at no cost to the Government if damage occurred during removal and/or installation. 3. PERSONNEL BADGES: All contractor employees will be issued an Altus AFB Contractor?s Badge, upon successful completion of a NCIC background check conducted by Altus AFB security forces, for unescorted access. Each contractor employee will provide the following information, but not limited to, while completing the Individual Application for Contractor Badge Business on Altus AFB, OK. This information is: full name to include first, middle, and last name, social security number, date of birth, address, driver?s license number and state of issue, number of years resident of the State of Oklahoma and Jackson County. This information will be protected under the procedures listed under the Privacy Act of 1974. Those contractor employees whose background checks reveal adverse criminal history may be denied access to Altus AFB. 4. FURNITURE MOVING: The contractor shall be responsible for the removal and reinstallation of the following furniture type items: desks, bookcases, cabinets, credenzas, tables, boxes, chairs, trashcans, and dormitory furnishings. Contractor will not be responsible for the moving of computer equipment, vending machines, personal items, safes, pool tables, or wall hangings. Furniture moved by the contractor shall be returned to its original location. 4.1. SYSTEM/MODULAR FURNITURE: Defined as furniture set in square/cubicle patterns with desks/tables/cabinets built integrally into or hung onto the wall system. The contractor shall only lift and reset System and Modular Furniture to the extent necessary for tucking carpet tile under walls/legs. Contractor will not disassemble/reassemble modular/system furniture.
