Description
This Sources Sought is a revision to REFERENCE NUMBER ? F09607-03-MEDICAL TRAINER. The 347th Contracting Squadron/MSCB is seeking sources to procure Medical Training in order to initiate a Blanket Purchase Agreement. FOR INFORMATION PURPOSES: The Standard Industrial Classification Code is 8299. The North American Industry Classification System is 611710. The procurement is a total small business set-aside. The Defense Priorities and Allocations System (DPAS) rating is N/A. Interested offerors must be CCR registered. Information concerning CCR registration requirements may be viewed via the Internet at http://www.ccr.gov or by calling the CCR Registration Centers at 1-888-227-2423. The service provided shall be in accordance with the Statement of Work. Price quote submissions shall reflect the Statement of Work description. THE STATEMENT OF WORK FOR EMERGENCY MEDICAL TRAINING IS AS FOLLOWS - 1. DESCRIPTION OF WORK: FURNISHING ALL INSTRUCTORS, EQUIPMENT, MATERIALS, AND RELATED ITEMS NECESSARY FOR INITIAL PARAMEDIC CERTIFICATION AND RECERTIFICATION FOR BASIC, INTERMEDIATE AND PARAMEDIC LEVEL EMERGENCY MEDICAL TRAINING AND BASIC LIFE SUPPORT, ADVANCED CARDIAC LIFE SUPPORT AND CONTINUING MEDICAL EDUCATION HOURS FOR THE 38TH RESCUE SQUADRON PERSONNEL IN ACCORDANCE WITH THE DEPARTMENT OF TRANSPORTATION (DOT) AND THE NATIONAL REGISTRY OF EMERGENCY MEDICAL TECHNICIANS (NREMT) STANDARDS. 2. SCOPE: THE FOLLOWING OUTLINE ESTABLISHES THE WORK TO BE PERFORMED (THE COMBINATIONS AND/OR VARIATIONS TO BE DETERMINED BY THE 38TH RESCUE SQUADRON, NCOIC, PARARESCUE MEDICAL TRAINING OR DESIGNATED REPRESENTATIVE). ALL TRAINING MUST MEET THE STANDARDS SET FORTH BY THE DOT/NREMT. 24 HR - TRAINING COURSE FOR BASIC EMERGENCY MEDICAL TRAINING RECERTIFICATION 36 HR ? TRAINING COURSE FOR INTERMEDIATE EMERGENCY MEDICAL TRAINING RECERTIFICATION 48 HR ? TRAINING COURSE FOR PARAMEDIC RECERTIFICATION PROVIDE BASIC LIFE SUPPORT (BLS) AND ADVANCED CARDIAC LIFE SUPPORT (ACLS) INITIAL CERTIFICATION AND RECERTIFICATION BASED ON THE AMERICAN HEART ASSOCIATION (AHA) STANDARDS. PROVIDE CONCURRENT 24 HRS, 36 HRS, AND 48 HRS OF CONTINUING MEDICAL EDUCATION CONSISTENT WITH DOT/NREMT STANDARDS PROVIDE ALL NECESSARY DOCUMENTATION THAT CONSISTS OF COURSE COMPLETION CERTIFICATES AND REGISTRATION CARDS FOR COMPLETED COURSES. 3. LOCATION: CONTRACTOR WILL CONDUCT REQUIRED TRAINING AT MOODY AFB, GEORGIA IN A PROVIDED CLASSROOM, OR PROVIDE AN ACCEPTABLE LOCATIONS FOR STATED TRAINING TO BE ACCOMPLISHED. 4. TIMES/DATES: CLASSES WILL BE CONDUCTED AS REQUESTED, BASED ON TDYs AND DEPLOYMENT SCHEDULES, WITH APPROXIMATELY TEN PERSONNEL PER CLASS. THE CONTRACTOR WILL BE GIVEN NO LESS THAN 14 DAYS NOTICE TO ESTABLISH TRAINING DATES. GOVERNMENT RESERVES THE RIGHT TO SPECIFY A SHORTER NOTICE TIME FRAME IF NECESSARY. 5. PERSONNEL: THE CONTRACTOR SHALL PROVIDE ALL INSTRUCTORS AND TEACHING MATERIALS COMMENSURATE WITH A WORKING KNOWLEDGE OF THE CONTINGENCY PARARESCUE OPERATIONS PROGRAM. ALL WORK PERFORMED SHALL BE UNDER THE SUPERVISION OF AN INSTRUCTOR WHO MEETS ALL DOT/NREMT/AHA INSTRUCTOR STANDARDS. SUBMISSION OF CERTIFICATION DOCUMENTATION SHALL BE MADE AVAILABLE UPON 347TH CONTRACTING OFFICER REQUEST. 6. SCHEDULE: ALL TRAINING WILL BE COORDINATED WITH THE 38TH RESCUE SQUADRON AND THE CONTRACTING OFFICER OR THEIR DESIGNATED REPRESENTATIVES. EACH ORDER WILL CONSIST OF A WRITTEN REQUEST FROM THE 38TH RESCUE SQUADRON SPECIFYING THE COMBINATIONS AND/OR VARIATIONS OF TRAINING REQUIRED DURING THE SESSION. THE CONTRACTOR WILL INITIATE INSTRUCTION ON A DESIGNATED SCHEDULE. ARRANGEMENTS WILL BE MADE BETWEEN THE GOVERNMENT AND CONTRACTOR REPRESENTATIVES TO ALLOW FOR INSTRUCTION DURING NON-DUTY HOURS. DUTY HOURS ARE CONSIDERED 7:30 AM ? 4:30 PM EST. IF APPLICABLE, HOLIDAY AND WEEKEND RATES SHALL BE INCLUDED IN THE QUOTE. 7. QUALITY CONTROL: QUALITY CONTROL IS THE JOINT RESPONSIBILITY OF THE 38TH RESCUE SQUADRON?S NCOIC, PARARESCUE MEDICAL TRAINING AND THE CONTRACTING OFFICER OR THEIR DESIGNATED GOVERNMENT REPRESENTATIVES. 8. INSPECTION: THE CONTRACTOR IS LIABLE FOR ALL DAMAGES TO GOVERNMENT PROPERTY. THIS INCLUDES BUT IS NOT LIMITED TO: FUNITURE, COMPUTER SYSTEMS, CHAIRS, CABINETS, OFFICE EQUIPMENT, ETC. THE CONTRACTOR MUST INFORM THE CONTRACTING OFFICER OF ANY DAMAGES TO GOVERNMENT PROPERTY. FOR TRAINING CONDUCTED AT MOODY AFB, GA GOVERNMENT PROPERTY AVAILABLE FOR USE BY THE CONTRACTOR IN THE CLASSROOM INCLUDES: COMPUTER(S), POWERPOINT PROJECTOR, TV AND VCR. THE GOVERNMENT REPRESENTATIVES WILL CONDUCT A FINAL AUDIT OF COURSE COMPLETION REQUIREMENTS (ON EACH COURSE). INSPECTIONS WILL BE ACCOMPLISHED BY THE 38TH RESCUE SQUADRON NCOIC, PARARESCUE MEDICAL TRAINING, OR THEIR DESIGNATED REPRESENTATIVES. THE 347TH CONTRACTING OFFICER RESERVES THE RIGHT TO ATTEND THE INSPECTIONS. THESE INSPECTIONS WILL BE CONDUCTED AT THE CONVENIENCE OF THE GOVERNMENT, BUT WITH COORDINATION OF THE CONTRACTOR. 9. BASE ENTRY PASSES: THE CONTRACTOR SHALL COORDINATE WITH THE CONTRACTING OFFICER OR THEIR DESIGNATED REPRESENTATIVE AND THE 347TH SECURITY FORCES PASS AND ID REGISTRATION TO OBTAIN THE NECESSARY BASE ENTRY AUTHORIZATION FOR ALL INSTRUCTORS. The sources sought proposal shall include the REFERENCE NUMBER (FA4830-03-MEDICAL TRAINER), NAME, ADDRESS, TELEPHONE NUMBER OF OFFEROR, PRICE, AND GSA NUMBER (if applicable).
