Description

This is a combined synopsis and solicitation for the following commercial items: 2 each: AMSCO Reliance 333i Washer/Disinfector without load lifting door part #MB06-04, 2 each: 2-level instrument Rack part #MB00-0015, 4 each: Freestanding Panels part #MC00-0012, 2 each: Washer Countertop part #MB00-0017, Installation Kit part #MB00-0037, 2 each: Klenzyme part #1673-BX, 2 each: Renu-Klenz part #1777-BX and Hinge Free part #1031-BX or equal. This synopsis and solicitation is prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quote (RFQ). Submit written offers (oral offers will not be accepted), on RFQ FM560632620600. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-14 and Defense Acquisition Circular 91-13. FOB Destination: for delivery to 52 Medical Group/Medical Logistics, GEB. 61 US-Wohnsiedlung, D-54634 Bitburg, Germany. The following provisions and clauses apply to this acquisition: FAR 52.211-6 Brand Name Or Equal, FAR 52.212-1 Instructions to Offerors- Commercial Items, FAR 52.212-2 Evaluation-Commercial Items, FAR 52.212-3 Offerors Representations and Certifications- Commercial Items, FAR 52.212-4 Contract Terms and Conditions-Commercial Items and 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders- Commercial Items.
