Gov.Works is issuing this request for proposals on behalf of the Office of Public Health and Science, an Office within U.S. Department of Health and Human Services which intends to competitively award a contract to provide respirator fit testing and respirators purchase for members of the Commissioned Corps Readiness Force and HEREBY SOLICITS CAPABILITY STATEMENTS from parties interested in proposing on this requirement.. PLEASE READ THIS ANNOUNCEMENT CAREFULLY. INTRODUCTION. The Government intends to award a combination of fixed price for equipment and time and materials-type contract for service and delivery. The period of performance for this project is 60 days from the effective date of the contract, with an estimated cost range of $100,000 to $120,000. The title of this project is CCRF Respiratory Protection Program. BACKGROUND. This Program shall apply to members of the Corps Readiness Force (CCRF) in the Intermediate and Advanced deployability categories and officers in the Commissioned Corps, who may be regarded as response assets in an emergency. These officers should be fit-tested annually with a disposable air-purifying respirator (e.g., N-95). It is felt that this level of respiratory protection will be appropriate for the majority of deployments and ensures that officers can be deployed fully protected against related risks associated with respiration of small particles. Additional levels of respiratory protection, based on an assessment of the respiratory risk, will not be addressed in this proposal. During the course of a deployment, officers may be exposed to biological agents that place them at risk for infection, injury, or illness. A comprehensive program is necessary that will meet the critical respiratory protection needs of deployed officers. A Respiratory Protection Program would apply to a variety of illnesses such as tuberculosis, influenza, or plague, and would provide guidance on the selection and wearing of appropriate respiratory protective devices for particulate contaminants. This contract will address the medical evaluation, training, fit-testing, and record-keeping of up to 1,000 CCRF members during fiscal year 2004. SCOPE OF WORK -- Deployed personnel must be adequately protected when responding to a public health need. Protecting deployed Commissioned Corps officers against infectious disease (e.g., SARS) or other contaminants (e.g., particulates). Qualified contractors must offer the following capabilities to be considered: Medical Clearance for Respirator Wear -- Every CCRF member who is under consideration to be fit-tested and trained to use an air-purifying respirator must receive medical clearance from a physician or other licensed health care professional employed by the contractor. Every CCRF member will complete an OSHA Respirator Medical Evaluation Questionnaire. The medical questionnaire will be completed on line by the CCRF member and submitted electronically to the contractor for review. The questionnaire will pose questions about current respiratory problems, physical conditions, hypertension, heart disease, musculo-skeletal problems, seizures, or any previous problems that may be encountered when wearing a respirator. Based on the results of the questionnaire, officers that indicate problems or who answer certain trigger questions may be required to have additional medical tests completed based on the recommendations of the licensed health care professional, such as a pulmonary function test utilizing spirometry. Spirometry testing, if required, will be provided by the contractor at each of the fit-testing sites. Officers, who are medically cleared by the contractor, either initially, or after their successful completion of spirometry, will proceed to training and fit- testing. EDUCATION PROGRAM -- The contractor shall ensure that respiratory program participants are provided information on-line related to risk assessment, disease states to be considered, respirator protection elements, medical clearance, and the appropriate use of personal protective equipment including limitations. Specifically, the training must include a discussion of the OSHA Respirator Standard (29 CFR 1910.134), proper use of respiratory protection devices, limitation(s) of the chosen respirator, donning and fit-checking procedures, and cleaning and/or disposal procedures. RESPIRATOR SELECTION AND AVAILABILITY - The disposable N-95 Respirator is a particulate respirator or "air-purifying respirator" because it cleans air by filtering particles as you breathe. These respirators only protect against particles, not gases or vapors. Airborne biological agents such as bacteria or viruses are particles, and as such, many can be filtered by particulate respirators such as the N-95. The N-95 respirator is the current respirator of choice for protection against tuberculosis. Additionally, it is believed by the CDC and other public health entities that the N-95 respirator will also provide the appropriate, minimum level of respiratory protection, against SARS for health care workers. There are many styles of N-95 respirators, and in the experience of knowledgeable environmental health officers, no single style of N-95 will be appropriate for everyone. A comprehensive, effective, and compliant respiratory protection program requires the availability of appropriate respirators to provide good fit for all participating individuals. This can only be accomplished through the provision of multiple models and sizes of required respirator types. Several models and sizes of respirators commonly found to result in successful fit-tests, as well as lesser quantities in appropriate numbers and sizes for alternative respirators will be furnished by the contractor, and shall be available at fit-testing sites to fit-test officers. SITE IDENTIFICATION - The CCRF will provide the contractor with a list of cities, and will provide appropriate space at Federal facilities for the contractor to provide fit-testing. Those cities are: Albuquerque, Anchorage, Atlanta, Boston, Butner, NC, Cincinnati, Dallas/Ft Worth, Gallup, NM, Los Angeles, Phoenix, Rockville, MD, Shiprock, NM, Washington, DC. RESPIRATOR FIT TESTING - The contractor shall provide respirator fit-testing via the quantitative method. Quantitative fit-testing occurs when a negative pressure respirator is placed on a participant and connected to a device which quantitatively determines whether any leakage has occurred. One such device is a Portacount, which is manufactured by TSI, Inc, and is probably the most commonly used. All CCRF members who have been medically cleared and have received appropriate education as described above, will be quantitatively fit-tested by the contractor. After successful fit-testing, CCRF members will be given the respirator with which they were fit tested. All personnel, equipment, and respirators necessary to complete the medical evaluation, training, fit-testing, and record-keeping shall be furnished by the contractor. PROGRAM RECORD KEEPING -- Because the sponsor retains some responsibility for ensuring that deploying officers are properly educated, medically cleared, fit-tested, tested annually, and medically followed over time, there must be an ongoing record keeping program. The contractor's records associated with medical evaluation, the results of any additional testing, training, fit-testing, and the actual respirator brand, model, and size that is successfully fitted for each officer shall be submitted by the contractor to the CCRF, and maintained by the CCRF. The contractor shall fully observe requirements for medical confidentiality of CCRF members. However, the Respiratory Protection Program Manager at CCRF will be provided with a copy of all related medical, training, and fit-testing records for each participant. BULK RESPIRATOR PURCHASE -- The contractor shall supply up to 20,000 respirators to the CCRF which shall correspond in brand, model, and size to those fit-tested for the CCRF membership. Approximately 2,000 respirators, in representative brands, models, and sizes as mentioned above, will be immediately shipped to CCRF headquarters. The remainder of the bulk purchase shall be housed at the contractor's warehouse or that of a bonded middleman for immediate overnight express shipment to the site of any deployment that might involve respiratory risk to the officers who deploy. This option of utilizing the contractor's resources for overnight shipment of respirators must be available. . HOW TO RESPOND: In order to compete for this requirement, you MUST demonstrate that you are qualified to perform the work by providing, by November 25, 3:00 pm EST an original and three (3) copies of a Capabilities Statement detailing (1) your key personnel (those who would have primary responsibility for performing and /or managing the testing program) with their qualifications and specific experience; (2) your organizational experience; (3) facilities; and (4) specific references including contract number and project identifier & description, period of performance, dollar amount, client identification with the point of contact & telephone number and Email address for previous work of this nature that your key personnel or organization has performed within the last four years. If you believe the Government will find derogatory information as a result of checking your past performance record, please provide an explanation and any remedial action taken by your company to address the problem. YOUR CAPABILITIES STATEMENT WILL BE EVALUATED BASED ON THE FOLLOWING FACTORS, with the importance of factors A and B weighted as indicated. (A) Experience and Commitment of Key Personnel. (1) Education, professional credentials, knowledge, and publications in relevant disciplines. (2) Expertise, education, and experience to carry out the specific tasks described above. (3) Adequate facilities to provide housing/storage for units. (B) Past Performance. This factor addresses your demonstrated ability to carry out the tasks efficiently and produce quality deliverables in a timely fashion as demonstrated by the following: (1) Ability to adhere to budget parameters and carry out the tasks of described below under specific scheduling deadlines, while delivering high-quality products. (2) The degree of comparability of past projects to the current project, including number, complexity, scope, size, duration, and dollar value. (3) Demonstrated degree of mutually beneficial cooperation between your key personnel and its clients. Qualifications of any subcontractors or partners will also be considered in these respects. FOLLOWING REVIEW OF ALL CAPABILITIES STATEMENTS, we will establish a list of those offerors GovWorks has evaluated as most qualified to perform the work. All offerors will be informed of the results of the evaluation, including an assessment of their strengths and weaknesses. A written request for proposal (RFP) will then be forwarded to those offerors deemed most qualified, instructing them as to the written proposal..
