Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. Solicitation FQ5004330101 is hereby issued as a Request for Quote. This solicitation document and all incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-06. This acquisition is 100% set-aside for small businesses under NAICS code 811490, which has a small business size standard of $6.0 million. The requirement consists of the following, (CLIN #, Quantity and Unit of Issue, Description;)Please provide quote accordingly. The 354th Civil Engineering Squadron requires one-year elevator maintenance. See statement of work. STATEMENT OF WORK MAINTAIN AND REPAIR ELEVATORS 1. SCOPE: The contractor shall test, maintain and repair as needed elevators, and their components and subassemblies, at Eielson Air Force Base Alaska in accordance with the latest edition of the American National Standard Safety Code for Elevators and Escalators (ASME A17.1), including fluids. Elevators shall be maintained in a safe and fully operational condition, or taken out of operation pending service. The elevator contractor and employees shall be highly skilled and show proof of experience in the complete maintenance and repair of elevators. 2. MAINTENANCE and INSPECTIONS: The contractor shall perform routine maintenance and periodic inspections as required in ASME A17.1. 3. REPAIR The contractor shall repair elevators as required. Notification of necessary repairs may come from contractor employees during routine maintenance and inspections, or from government personnel in the event of an elevator malfunction. 4. HOURS OF OPERATION The contractor shall perform service during normal duty hours, 0700 to 1700 Monday through Friday. The contractor is not required to provide service on Saturdays, Sundays, or federal holidays. Upon notification of an elevator in need of repairs, the contractor should normally respond by the next workday. The contractor shall not be required to respond to elevator emergencies. Emergency situations occurring shall be dealt with by Eielson AFB emergency services personnel. 5. MAINTENANCE CERTIFICATIONS The contractor shall conspicuously post a suitable placard in each elevator with the following minimum information: a. This elevator is inspected and serviced by: (contractor). b. In the event of malfunction or trouble, call 377-2100. In case of emergency, call 911. c. Dates of recent inspections and service with the technician's name. d. Date due for next inspection or service. e. Dates due for periodic inspections. 6. PARTS The government will reimburse for all parts in excess of $100.00. The contractor shall submit appropriate invoices prior to reimbursement. All contractor-furnished parts and components furnished in the performances of this contract shall be guaranteed for a period of one year (or manufacturer's guarantee if longer than one year) from the date of installation. 7. PRICING The fixed price established in the schedule is all-inclusive of all direct and indirect (labor, travel, overhead, general and administrative expense, and profit) costs, except those specified as separate items in the schedule. 8. HAZARDOUS MATERIALS AND WASTES The contractor shall be responsible for the replacement, clean up and disposal at an off-base site of all elevator fluids, including hazardous wastes.
