Description

NA The Dept of Forensic Toxicology Drug Testing Lab, Tripler Army Medical Center, Hawaii has a requirement for (4) types of barcode labels. Qty and size vary. Delivery will on-call and payment by Govt Purchase Card. Interested source shall submit a bid sample for evaluation and testing to determine if it meets the Governments minimum needs. Bid samples shall be compatible for testing on Government-owned Monarch 9840 and Zebra Eltron TLP3742 printers. Bid samples shall be received with offer and posted no late r than the bid closing date. See FAR 52.214.20. Offeror shall provide a list of (3) recent sales to Government and/or commercial concerns with its offer. Give name, title, organization and phone number of person who is in the position to evaluate your a bility in the area of contract performance, responsibility, timeliness and reasonable and cooperative behavior and commitment to customer satisfaction. The Govt anticipates to award a Firm Fixed-Price IDIQ contract for the base period 1 Dec 03 or date of a ward - 30 Sep 04 with (3) year options. The requirement is 100% Small Business Set-Aside.
