Description
This is a COMBINED SYNOPSIS SOLICITATION for the repair of a John Deere 544H Loader, a commercial item prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. Request for Quotation HAQ042001 incorporates provisions and clauses in effect through Federal Acquisition Circular 2001-1. The North American Industry Classification System (NAICS) code is 811310, with a small business size standard of $6 million. This is a firm-fixed price contract for the Bureau of Land Management, Roseburg District, Oregon. This solicitation has one line item with quantities as follows: Item 1 - Repair of John Deere 544H Loader (Government will deliver and pickup the loader), 1 lump sum. EVALUATION FOR AWARD: Award will be made to the responsible firm whose quote is most advantageous to the Government, price and past performance considered. An evaluation factor of $1.66 per one mile increment over 200 miles will be added to the Item price to compute the true cost to the Government. Mileage will be measured on a round trip basis from contractor's facility to the Sutherlin Maintenance Shop (SMS) using the official Oregon Highway Map. For example: Contractor's facility is 240 miles from SMS, therefore, 80 miles at $1.66 = $132.80 will be added to the Item price for evaluation purposes. TECHNICAL SPECIFICATIONS: Repair damages caused by a rollover accident. Due to the built in rollover protection in the cab, the cab shall be replaced. Use John Deere replacement parts or equal. The loader will be available for viewing, by appointment only, December 1 through December 5, 2003 at the Bureau of Land Management, Sutherlin Maintenance Shop, 391 S. Calapooia, Sutherlin, OR 97479. For an appointment, contact Bruce Sconce, Roseburg District Office at 541-464-3470. PERFORMANCE AND ACCEPTANCE: Loader repairs shall be completed within 45 calendars days from receipt of the loader. The Government will deliver and pickup the loader. Inspection will be visual. Acceptance will be made upon the Government's determination that the requirements of these specifications have been met. Contractor shall provide a detailed summary of work with an associated cost estimate for loader repairs with its quote. Contractor will be paid for the actual cost of services performed based on this estimate. If additional repairs are required during performance of the work, prices will be negotiated in accordance with the FAR clause 52.212-4. APPLICABLE FAR CLAUSES AND OTHER PROVISIONS: The following FAR provisions apply to this RFQ and are incorporated by reference: 52.204-6 Contractor Identification Number -- Data Universal Numbering System (DUNS) Number; 52.212-1 Instruction to Offerors - Commercial Items; 52.212-2 Evaluation - Commercial Items; 52.212-3 Offeror Representations and Certifications -- Commercial Items (Note: offerors must include a copy of the provisions at 52.212-3 with quote -- this can be obtained at http://www.arnet.gov/far); 52.212-4 Contract Terms and Conditions - Commercial Items; 52.212-5. Required to Implement Statutes or Executive Orders - Commercial Items Alternate II (including 52.203-6, 52.2l9-8, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-3, 52.232-33), 52-222-41 Service Contract Act, 52.222-42, Statement of Equivalent Rates for Federal Hires, and 52.222-47, SCA Minimum Wages and Fringe Benefits Applicable to Successor Contract Pursuant to Predecessor Contractor Collective Bargaining Agreements). Offerors shall submit signed and dated quotes for Item 1, including the detailed summary of work and cost estimate as noted above. Offerors shall also provide the mileage from the SMS to the quoter's repair location. Quotes shall be submitted either on a SF1449 or letterhead stationery. Quotes shall be sent to Bureau of Land Management, Branch of Procurement (OR952), 333 S.W. First Avenue, 4th Floor, Portland, OR, 97204 COB (4:15 p.m. local time) on or before December 11, 2003.
