- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and is being issued as a Request For Quotation (RFQ) and a written solicitation will not be issued. This will be a Firm Fixed Price Contract, FOB Destination. The solicitation number is 549-04-RFQ-25. The NAICS code is 337127 with the size standard of 500 employees. The solicitation document, incorporated clauses and provisions are those in effect through Federal Acquisition Circular 2001-16 and Veterans Affairs Acquisition Regulation (VAAR), VA Acquisition Circular 26. The Contracting Activity located at the VA North Texas HealthCare System (VANTHCS), 4500 South Lancaster Rd., Dallas, TX 75216, for Naval Medical Center, located in Portsmouth, Virginia, requires Chapel furnishings for its facility. Chapel is located on the 2nd floor of Bldg. 215, Naval Medical Center. Items requirements are: LINE ITEM 0001 - Pulpit -1ea. ?Octagonal ? 45?H x 48? W x 20? D, LINE ITEM 0002 - Altar - 1 ea. ? 40? D ? must accommodate one (1) person, LINE ITEM 0006 - Lectern - 1 ea. - block-type pedestal versus a cross-member design with a slanted/tilted bookstand ? 46?H x 22? W x 19?D, LINE ITEM 0007 - Chancel Chairs -w/arms, 1ea. - 31 ?? H x 27 ?? W X 27? D, LINE ITEM 0008 - Chancel Chairs - w/o arms- 2 ea ? 361/2? H x 23? W x 23? W x 27? D ? hinged slanted top with pen holder ridge at the edge and card holders at the top back of unit, LINE ITEM 0011 - Chairs w/kneelers and book rack -150 ea. - Front-loading offset book rack, - 33.5?H x 20? D ? chairs will have double-dowel/double screw leg assembly, 3? of high density foam padding in the seat ? back will be contoured backrest. Chairs must have attached kneelers. Chairs will not have a ganging device, and will not be permanently affixed to the floor, with a fabric finish on the backrest as well as the seat. The chapel furniture will be constructed of wood with light to medium oak color stain. All pieces will be coordinated to match in style, color, and will have no inscription, no denominational symbols or markings. FABRIC requirements: fiber ? 100% Polyolefin, is to be high quality material, color selection: Navy. Fabric and foam must meet California code flame retardant requirements. Desired delivery is 60 days. Contractor shall provide all materials, tools, labor and transportation. Proposals are to include samples of wood and fabric. Contractor shall submit literature, warranty certification with samples of all materials to be used: stain, wood, fabric, and foam. Installation: Contractor shall notify the government 10 days prior to delivery and installation of furnishings. Contractor shall ensure manufacturing and installation of all furnishings materials shall be finished, splinter free at all cuts, edges, and surfaces shall be smooth and clean in accordance with industry standards. For efficiency and cost effectiveness, it is the VA?s intent to award a single PO to the one responsive and responsible bidder, whose offer is most advantageous to the government price and other factors considered.
