- This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number is FA8501-04-Q-0007. This solicitation is issued as a request for quotation (RFQ). The solicitation document, incorporated provisions and clauses are in effect through Federal Acquisition Circular 201-09. The North American Industry Classification System (NAICS) Code is 541612 and the small business size is $60M. This requirement is for 6 months after receipt of order. The solicitation contains CLIN 0001 for Advisory and Assistance Service (A&AS) for one individual (Management Analyst to provide workers' compensation investigative services. CLIN 0002 is for DATA. Offerors shall propose a firm-fixed price monthly rate for CLIN 0001. This requirement is set aside for small businesses. The period of performance will begin on or about 15 Dec through 15 Jun 04. Clause 52.212-1, Instructions to Offeror - Commercial Items applies to this acquisition and the addenda (Performance Work Statement or PWS) that may be accessed at http://pkec.robins.af.mil/pr/mssc/mssc.htm under Contracting with Individuals for Manpower Support, Solicitation F09650-04-Q-0007. INDIVIDUALS WHO HAVE PREVIOUSLY SUBMITTED PROPOSALS UNDER THE CONTRACTING WITH INDIVIDUALS PROGRAM NEED TO CONFIRM THEIR PREVIOUSLY SUBMITTED PROPOSAL. PREVIOUSLY SUBMITTED PROPOSALS MAY BE REVISED OR UPDATED. This requirement requires the service of one individual equivalent o a GS-343/12 Evaluation of offerors will be made on a best value basis. Evaluation factors, in descending order of importance, are experience, past performance, price, education and general considerations. See the source selection plan at the web site referenced above for a complete explanation of the evaluation process. Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes of Executive Orders-Commercial Items applies. The following clauses are incorporate by reference: FAR 52.212-1, 52.212-3, and 52.212-4. The following DFARS clause, 252.225-7001, Defense Priorities and Allocations Systems, applies (D0C9). The proposed contract is 100% set aside for small business concerns.
