- NA This is a combined synopsis/solicitation for a commercial service, prepared in accordance with the format in FAR Subpart 12.6, and supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quot es are requested; however, a written solicitation will not be issued. Solicitation number W911S8-04-T-0007 is applicable and is issued as a request for quotations. The solicitation and incorporated provisions and clauses are those in effect through Feder al Acquisition Circular 2001-03. The requirement is small business set-aside with a NAICS Code of 561740, SIC Code 7217, and size standard of 4 mil. The description of the commercial service is as follows: Statement of work Floor cleaning and Maintenance Bldg 3764, 3765, and 3766 BASIC SERVICES. The contractor shall provide all management, tools, supplies, equipment and labor necessary to provide one time cleaning services to shampoo all carpeting, strip, seal, and wax tiled floors in buildings 3764, 3765, and 3766 at Fort Lewis, WA. Work shall comply with Government and commercial standards. Tentatively, t he below described work is to be accomplished per the below described schedule. DESCRIPTION OF WORK: Each building consist of four separate wings, A, B, C, and D. The B, and C wings have eight one room apartments per floor. The A wing consist of eight one room apartments on the second and third floors and a day room with four one room apartments on the ground floor. The D wing also has eight one room apart ments on the second and third floor and a day room and admin area on the first floor. FURNITURE: All the one room apartments have furnishings that shall be removed during carpet cleaning operations. Furnishings shall be returned to their original position after carpet cleaning. CARPET CLEANING: All carpets shall be cleaned in accordance with standard commercial practices. After shampooing, the carpeted area shall be uniform in appearance and reasonably free of stains and discoloration. All baseboards shall be free of all cleaning solutions. Any items moved to clean carpets shall be returned to their o riginal location. 1.4 FLOOR STRIP, SEAL, AND WAX: All floors that are stripped, sealed, and waxed shall have a uniform glossy appearance. A uniform glossy appearance is free of scuffmarks, heel marks, wax build-up, and other stains and discoloration. A non-skid wax is r equired. Any items moved to necessitate cleaning shall be returned to it??????s original position. COMPLETION SCHEDULE: BLDG. 3764 1st Floor 8 through 12 Dec, 03. BLDG 3764 2nd Floor 12 through 16 Jan, 04. BLDG 3766 3rd Floor 8 through 12 Dec, 03. 1.6 WORK EXCEPTANCE: Final acceptance walk throughs will be conducted with a minimum of 4 hours notification. FIRM FIX PRICE CONTRACT SITE VISIT IS REQUIRED PRIOR TO QUOTE SUBMITTION PLEASE CONTRACT JACQUELINE EDGERTON FOR APPOINTMENT AT 253-966-3481 OR BILLY CALVERT AT 253-966-1733. Requirment is FOB Destination only. Activity is Public Works, Services Mangement Branch, Fort Lewis, WA 98433-9500. Any award resulting from this request for quotations will be made to a responsive, responsible quoter using a SF1449 form. Quotes will be evaluated on a based of Technical and best value. Award will be made on an all or none basis. Provision 52.212-1, Instructions to Offertory-Commercial Service, applies to this acquisition. An addendum to this provision is applicable, in which the term offer or (whenever it appears) shall be replaced with the term quoter. Quoter are required to submit completed copies of the provisions at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, with their quotes. Also, please provide Federal Tax ID Number FAR clause 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition and does not include any addenda.
