- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This procurement is conducted under simplified acquisition procedures in accordance with FAR Part 13. This announcement constitutes a formal Request for Proposals (RFP) and a written solicitation WILL NOT be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-16. RFP 246-04-00940 should be referenced on all bids. The applicable NAICS code is 621111 with a size standard of $8.5 million. The VA Medical Center, 1970 Roanoke Blvd., Salem, Virginia 24153 has a requirement for Ophthalmology Services. This procurement is to obtain the services of Retinal Clinic coverage located at the Salem VAMC, providing services to clinic patients with hospital Residents in attendance, supervising retina laser treatments, performing surgical procedures, performing vitrectomies, providing photodynamic therapy for macular disease, performing retinal detachment repair, instructing Residents on ophthalmic procedures, and providing surgical attendings coverage for ophthalmic plastic surgery in the Salem VAMC Operating Room. Services are required for the period January 1, 2004 up to and including December 31, 2004 with four (4) available one (1) year option renewal periods. Ophthalmology Service staff shall peform in accordance with the policies of Salem VAMC and shall have professional and legal responsibility to the patient and shall be required to achieve the standards of care for Ophthalmology Services, to include the following: Retinal Specialist: Contractor shall provide the services of a fully-trained Retinal Specialist, Board Certified in Ophthalmology, with a minimum of two (2) years Fellowship in Retina. Services shall include providing coverage at the Salem VAMC Retinal Clinic, providing services to clinic patients with hospital Residents in attendance; supervising retina laser treatments, performing surgical procedures, performing vitrectomies, providing photodynamic therapy for macular disease, retinal detachment repair, and instructing Residents on ophthalmic procedures. The Retinal Specialist must be credentialed and privileged with a local hospital, and shall submit documents to obtain staff privileges at the Salem VAMC as well. Certification in Basic Life Support (BLS) is a requirement. Estimated requirements for services: Coverage at Retinal Clinic ? one (1) day per week, forty-five (45) weeks per year, scheduled Wednesday 8:00 a.m. ? 4:00 p.m. Vitrectomy: four (4) to seven (7) per month, twelve (12) months per year. Photodynamic Therapy: four (4) to six (6) patients per year, three (3) to five (5) treatments per patient. Drugs will be furnished by Salem VAMC. Retina detachment repair ? one (1) to three (3) per year. Provide digital angiography unit and ophthalmic photographer as needed. Photographer must be fully trained in the operation of the unit and provide documentation thereof with proposal. Estimated requirements for services: four (4) to six (6) patients per year. Provide the services of an Ophthalmic Plastic Surgeon, Board Certified in Ophthalmology, with a minimum one (1) year Fellowship in Ophthalmic Plastic Surgery. Services are to provide surgical attending coverage for ophthalmic plastic surgery in the Operating Room at the Salem VAMC. The Ophthalmic Plastic Surgeon must be credentialed and privileged with a local hospital, and shall submit documents to obtain staff privileges at the Salem VAMC as well. Estimated requirements for services: one (1) day per month, twelve (12) months per year. The Contractor shall furnish to the Contracting Officer with their proposal a Certificate of Insurance which shall contain an endorsement to the effect that cancellation of, or any material change in the policies which adversely affect the interests of the Government in such insurance shall not be effective unless a 30-day written notice of cancellation or change is furnished to the Contracting Officer. Contracting with Parties Listed on the OIG List of Excluded Individuals/Entities: In accordance with the Health Insurance Portability and Accountability Act and the Balanced Budget Act of 1977, the Office of Inspector General has established a list of parties and entities excluded from Federal Health Care programs. Specifically, the listed parties and entities may not receive Federal Health Care program payments due to fraud and/or abuse of the Medicare and Medicaid programs. Offerors should note that any excluded individual or entity that submits a claim for reimbursement to a Federal health care program, or causes such a claim to be submitted, may be subject to a Civil Monetary Penalty (CMP) of $10,000 for ach item or service furnished during a period that the person or entity was excluded and may also be subject to treble damages for the amount claimed for each item or service. CMP?s may also be imposed against health care providers and entities that employ or enter into contracts with excluded individuals or entities to provide items or services to Federal program beneficiaries.
