- A General Information: Document Type: Combined Solicitation. Solicitation Number: F73SGL328302 Posted Date: 3 December 2003. Original Response Date: 10 December 2003 Current Archive Date: Classification Code: R - Professional, Administrative and Management Support. Set Aside: Total Small Business. Contracting Office Address Department of the Air Force, Direct Reporting Units, 10 MSG/LGC, 8110 Industrial Drive, Suite 200, USAF Academy, CO 80840-2315. Description: This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotations are requested and an additional solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ), solicitation number F73SGL328302, in accordance with FAR Parts 12 and 13. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-15. The North American Industrial Classification system (NAICS) number is 561421 and the business size standard is 500 employees, The proposed contract is a 100% set aside for small business concerns. LINE ITEM 0001 is for After-Hours Medical Telephone/Answering and Messaging Service for the United States Air Force Academy Hospital. LINE ITEM 0001 includes the following requirements: (1) Call and messaging service to be provided by the contractor as follows: a. Monday?s starting at 1630 hours ending at 0800 hours on Tuesday?s. b. Tuesday?s starting at 1630 hours ending at 0800 hours on Wednesday?s. c. Wednesday?s starting at 1630 hours ending at 0800 hours on Thursday?s. d. Thursday?s starting at 1630 hours ending at 0800 hours on Friday?s. e. Friday?s starting at 1630 hours ending at 0800 hours on Saturday?s. f. Saturday?s starting at 1400 hours ending at 0730 hours on Monday?s. g. 24-hour coverage for all Federal holidays. h. 24-hour coverage for approximately eight unscheduled weekdays during the calendar year. Operators shall instruct callers that for ?life threatening situations? the caller shall hang up and dial 911 or go to the nearest hospital emergency department. (3) If callers wish to speak with a physician, operators are required to record the callers name and contact information and page the on-call medical doctor within ten (10) minutes so he/she can contact the patient. (4) The contractor shall provide a primary and alternate point of contact name and telephone number, in writing, to 10MSG/LGCB/John Kurko for the purpose of discussing matters relating to this contract. The contractor shall insure the letter is received by USAFA not later than five workdays prior to contract start date. (5) The contractor shall be available via telephone to discuss support questions and issues daily from 0730 to 1630 hours excluding weekends and Federal Government holidays. (6) Personnel authorized to act on behalf of the contractor shall be knowledgeable in the area they are authorized to act upon. The contractor is responsible for insuring that contractor personnel/employees are able to resolve telephone issues and the reporting of the same to the 10MDG/SGOPF in a timely manner. (7) The contractor shall maintain a log or register of incoming telephone calls/messages. The contractor shall provide a monthly report to the 10 MDG/SGOPF listing the number of telephone calls/messages received and processed for the calendar month. The report shall reflect the above referenced information to include call duration, and be furnished to the 10MDG with the invoicing information at the end of the monthly billing cycle. The FOB for this RFQ is Destination. Quotations received with other than FOB Destination pricing may not be considered. The contract effective date is 3 February 2004 to 2 February 2005 with three (3) option periods. The following Federal Acquisition Regulations (FAR) clauses apply to this combined synopsis/solicitation and offerors must comply with all instructions contained herein: FAR 52.212-1, Instructions to Offerors-Commercial Items, no addenda applies: The provisions at 52.212-2-, Evaluation/Commercial Items applies to this acquisition. The Government will award a contract resulting from this solicitation that will be most advantageous to the Government, price and other factors considered. The government reserves the right to select a contractor at other than the lowest price. Quotes that are unrealistic in terms of price may be rejected. Price will be evaluated to determine its fairness and reasonableness as it relates to the service offered. The Government intends to evaluate quotations and make an award without discussions with proposed contractors. However the Government reserves the right to conduct discussions with prospective contractors if it is in the best interest of the Government to do so. Offerors shall include a completed copy of Representations and Certifications/Commercial Items, with its offer. A Representations and Certifications/Commercial Items document may be located at: www.farsite.hill.af.mil. The following clauses apply to this acquisition. The clause 52.212-4, Contract Terms and Conditions/Commercial Items with the following addenda applies to this solicitation: 52.247-34, FOB Destination (November 1991). The clause at 52.212-5, Contract Terms and Conditions required to Implement Statutes or Executive Orders-Commercial items applies to this acquisition. Additional FAR clauses cited in the clause applicable to this acquisition are (b) (2) 11, 12, 13, 14, 15, 16, 21, 23 and 24. DFARS 252.212-7001, Contract terms and Conditions required to implement Statutes of Executive Orders applicable to Defense Acquisitions of Commercial Items (July 2002) applies to this acquisition.
