INTRODUCTION: The 11th Contracting Squadron, (LGCN) is requesting Firm Fixed Price proposals be submitted in support of purchase request F9D0SA43170100, to provide a physical fitness facility (GYM) for the use of its people, at no cost. The National Security Space Architect ( NSSA) Facility will purchase a group membership covering 30 people at a gym within a 5 minute (driving distance) of the NSSA Facility. The offerors proposal must include all data and information requested herein and be submitted in accordance with these instructions. The offeror shall be compliant and ensure that all work performed is within the scope defined in the attached Statement of Work (SOW). This announcement constitutes the only solicitation for proposals. A separate written solicitation will not be issued. This acquisition is open for full and open competition. Statement of Work for Physical Fitness Facility (Gym) Group Membership (As of December 2003) 1. Introduction. The National Security Space Architect (NSSA) is a federal government organization located mailing address P.O. Box 222310 Chantilly, VA 20153-2301. The NSSA employees include active duty military personnel. The Department of Defense requires these personnel to be physically fit. 2. Program and Performance Objectives The NSSA desires to provide a physical fitness facility (gym) near by for the use of its people, at no cost to those people. The NSSA will purchase a group membership covering 30 people at a gym within a 5 minute (driving distance) of the NSSA Facility. This group membership will provide the same access to facilities and services that the Gym offers commercially to the general public. 3. Scope 3.1. Description of Services 3.1.1. The Contractor shall provide NSSA with a group membership covering 30 people at one time. The group members shall have same access to facilities and services that the Gym offers through standard commercial membership, such as the use of locker rooms, showers, dry saunas, steam rooms, towel service, swimming pool (group members will work around the pool aerobics schedule), whirlpool, aerobic exercise equipment, strength training equipment, flexibility training equipment, multi-purpose rooms, personal trainers (after the 2 free sessions as a new group member, a fee will apply, which must be paid by the individual), and classes. 3.1.2. Transfer procedures - After the initial assignment of 30 people to the NSSA's group membership, the NSSA will pay the contractor a transfer fee for every old member replaced by a new member in accordance with the Purchase Order. NSSA reserves the right to replace an NSSA group member as long as the total does not exceed 30 transfers per year. A single membership slot may have more than one transfer during the year, but the total number of transfers during the year for the NSSA group membership will not exceed 30. 3.1.3. Termination procedures - The contractor shall waive its normal 30-day cancellation notice for existing Gym members who will transfer to the NSSA group membership. Changing private monthly memberships to the NSSA group membership must occur at the beginning of the month, as the Gym will not prorate membership fees for periods less than a month. 3.1.4. If a new person is added to the NSSA group membership, the person will process into the Gym's membership following the Gym's normal procedure, except that the Gym will not require any credit card or bank account information. 3.2. Service Delivery Schedule 3.2.1. Service will start on first day of the month following contract award. 3.3. Disclaimer ? The government assumes no responsibility or liability for any services rendered outside of the scope of the Purchase Order. 3.4. Government Furnished Property and Services None. 3.5. General Information 3.5.1. Deliverables None. 3.5.2. Security None. 3.5.3. Period of Performance One full year (1 January 2004 ? 31 December 2004). 3.5.4. Place of Performance The physical fitness facility. 3.5.5. Hours of Operation The normal hours of the physical fitness facility. 3.6. Appendices None. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-16. Proposals will be evaluated on a lowest priced technically acceptable basis. This notice does not obligate the Government to award a contract nor does it obligate the Government to pay for any proposal preparation costs. The Government will award a contract resulting from this solicitation to the responsive and responsible offeror who?s offer fulfills the requirements and has the lowest priced technically acceptable offer. The Government reserves the right to cancel this synopsis/solicitation, either before or after the receipt of proposal(s), with no obligation to the offeror by the Government. The Contractor must be registered in the Central Contractor Registration (CCR) system to be eligible for an award. The website is www.ccr.gov. Proposals are due 15 DEC 2003 at 3:00 PM Eastern Standard Time (EST) to 11 CONS/LGCN, 110 Luke AVE, Ste 250, Bldg 5681, Attn: William Underwood, Bolling AFB, DC 20032-0305 by mail. An original and one copy of the cost and technical proposal shall be provided. Representations and Certifications (FAR 52.212-3 and DFARS 252.212-7000) shall also be provided.
