Description
The Center for Disease Control and Prevention in collaboration with the Division of International Health and the Division of Public Health intends to purchase on a sole source basis from its authorized Dell dealer in El Salvador, IMAGENES DIGITALES, RAF S.A. DE CV., a National Electronic Disease Surveillance and Informatics System. IMAGENES DIGITALES, RAF S.A. DE C.V., is the only known source capable of providing the equipment and on-site support in El Salvador, and capable of providing the language requirement ?Spanish language? support under this project. The purpose of the Surveillance System is to organize and rapidly communicate public health data from the Basic Integrated Health Systems (BIHS) to the central level of the Ministry of Health for decision-making, planning public health interventions, monitoring disease trends, and program evaluations. The first phase of the project is the development and implementation of the surveillance systems, the purchase of 28 computers with printers and UPS units is required. Computers, printers, and UPS units are to be installed in 25 health units and 3 SIBASIS of La Paz, El Salvador. The computer equipment shall be delivered and installed in La Paz within one week after award of the purchase order. Due to unique computer specifications and requirements (Spanish Operating System, Spanish MS Office, Spanish keyboard, Norton Antivirus compatible with Spanish OS, and next day onsite support service by Spanish-speaking technicians The computer equipment and on-site service support will continue for a 3-year period. The following list includes the required equipment: Pentium IV processor, 2.20 GHz, 512K Cache Memory 512 MB 40 GB Hard Drive Network card 10/100 kbps with connector to UTP cable 24 x 10 x 40 x CD ROM RW with all accessories 56 Kbps internal Modem with manual and drivers Mouse/Mouse pad 3.5 Floppy drive Spanish-speaking users keyboard 17? Color Monitor Parts: 2 Serial, 1 parallel, 2 USB, reinstalled software with licenses: Spanish Windows 2000, Spanish MS Office (with Word, Excel, Power Point, Access 2002) Norton Antivirus (with upgrade for up to 2 years) compatible with Spanish OS Next day onsite support services warranty by Spanish-speaking technicians. Color Printe: Preferred Inkjet printer Print speed up to 7 ppm in color Print speed up to 8 ppm in black With parallel and USB ports With parallel cable, drivers and manual. UPS Units: 500 VA minimum with automatic voltage regulations Minimum 4 outlets with surge protection Manual included. The Government intends to evaluate offers and award a contract without discussion with Offeror?s providing capabilities required under this project. Therefore, the initial offer should contain the offeror?s best terms from a technical and price standpoint. THIS PROCUREMENT IS AN ALL OR NONE REQUIREMENT. The North American Industry Classification (NAICS) Code is 513340, and the Size Standard is 11M. Interested vendors capable of furnishing the equipment and service listed above shall submit two (2) copies of the quotation to the address listed below. The quotation shall include the itemized price, prompt payment terms, the F.O.B. Point (Destination or Origin), Taxpayer Identification Number (TIN), Dun & Bradstreet Number (DUNS) and descriptive literature of the product quoted. The quotation must reference Solicitation Number 2004-Q-01131. Quotations will be due no later than close of business Wednesday, December 17, 2003. All responsible sources may submit inquiries. Quotations must be submitted in writing to the attention of Maria Nichols. Email quotations will be accepted if dated and signed by an authorized company representative to mkn2@cdc.gov. Interested party that believes they can provide the detailed requirement as stated above, may submit a ?Statement of Capabilities? and other information detailed in writing to CDC. This action is proposed pursuant to 41 U.S.C. 253(1) and under the authority of FAR Part 6.302-1. The requirement will be procured as a commercial item buy in accordance with FAR 12.102(c) under the test program. A determination by the Government not to compete this proposed required based on responses to this notice is solely within the discretion of the Government. THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT INCLUDED IN SUBPART 12.6, as supplemented with additional information included in this notice. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTES ARE BEING REQUESTED AND A WRITTEN REQUEST FOR QUOTE WILL NOT BE ISSUED. The following provisions apply to this acquisition: 52.212-1 ?Instruction to Offeror?s? ? Commercial Items (MAR 2000) applies to this acquisition; 52.212-2, Evaluation ? Commercial Items (JAN 1999); the evaluation criteria stated in paragraph (1) of this provision are as follows: (1) Technical Capability of the proposed system to the meet the requirements stated herein, and (2) price. The clause 52.212-4, Contract Terms and Conditions Commercial Item (MAY 1999) applies to this acquisition. The following FAR Clauses are hereby added to this clause: 52.247.35 F.O.B. Destination; the clause at 52.212-5 Commercial items applies to the acquisition. The following clauses listed in paragraph (b) of 52.212-5 apply to this acquisition: (1), (3), (5), (7), (9), (11), (12), (14), (15), (17), (18), (18), (20) and (24).
