- Provide all personnel, equipment, and supplies necessary for non-emergent patient transportation for the John D. Dingell Department of Veterans Affairs Medical Center, 4646 John R. Street, MI 48201. Contractor shall perform these services for patients residing primarily within Metro Detroit City Limits. THERE WILL BE A PRE-AWARD CONFERENCE / WALK THROUGH FOR THIS PROCUREMENT ON TUESDAY, DECEMBER 16, 2003 AT 10:00 AM LOCAL TIME IN ROOM B4213, AT THE JOHN D. DINGELL VA MEDICAL CENTER, 4646 JOHN R. STREET, DETROIT, MICHIGAN 48201. THE MEDICAL CENTER IS LOCATED ON JOHN R STREET, BETWEEN CANFIELD AND HANCOCK. BRUSH ST RUNS BEHIND THE BUILDING AND FORREST ST RUNS DIRECTLY TO FRONT OF THE BUILDING. IT IS EASIEST TO PARK ON THE HANCOCK STREET (RED) ENTRANCE THEN TAKE ELEVATORS IN MAIN BUILDING TO FORTH FLOOR (RED SECTION) ROOM B-4213. CALL EXT 3006 OR 3354 IF NEED FURTHER DIRECTIONS. COPIES OF RFP 553-04-04 are now available on this website. This procurement is set aside 100% for small business concerns. The North American Industry Classification System (NAICS) for this procurement is 485991, Special Needs Transportation. The size standard is 6 million average annual receipts for the past three years. Projected period of service is from date of award through January 31, 2005, with three pre-priced option years. Request for Proposal is available on this website. ANY CHANGES TO THE WALK THROUGH (or to the solicitation) WILL BE POSTED ON THIS WEBSITE so it is advised that prospective contractors check the website prior to coming to VA and prior to submitting a proposal. PLEASE PLAN TO ARRIVE EARLY for the walk-through. TIME FOR A QUESTION AND ANSWER SESSION WILL BE ALLOWED. If unable to pull RFP 553-04-04 off website, requests for proposals shall be submitted in writing to the John D. Dingell Department of Veterans Affairs Medical Center, Acquisition Management (001R-A), 4646 John R. Street, Detroit MI 48201. Written requests may also be submitted via facsimile at (313) 576-1077. Telephone or internet requests will not be accepted. All requests must reference solicitation RFP 553-04-04. Proposals are due by 4:00 pm, local time, Friday, January 2, 2004. Proposals received after due date will not be considered. Contractors are responsible to verify documents have been received in the contract office. IN ACCORDANCE WITH THE PROVISIONS OF FEDERAL ACQUISITION REGULATION 19.502-2, AWARD FOR THIS SERVICE WILL BE MADE ON A COMPETITIVE BASIS TO ELIGIBLE AND RESPONSIBLE SMALL BUSINESS CONCERNS, PROVIDED THAT A MINIMUM OF TWO COMPETITIVE OFFERS IN ACCORDANCE WITH THIS SOLICITATION, ARE RECEIVED FROM ELIGIBLE SMALL BUSINESS CONCERNS. IF A MINIMUM OF TWO ELIGIBLE AND RESPONSIBLE OFFERS FROM SMALL BUSINESS CONCERNS ARE NOT RECEIVED, AWARD WILL BE MADE ON THE BASIS OF FULL AND OPEN COMPETITION FROM AMONG ALL RESPONSIBLE BUSINESS CONCERNS THAT SUBMIT AN OFFER.
