- Amendment 0001 Solicitation No: 161-03-0039 Page 2 0f 2 1. The following FedBiz Ops notice replaces the original notice posted on October 24, 2003. DESCRIPTION OF WORK: Santa Rosa and Sells Transient Quarter Roof and Equipment Replacement Santa Rosa and Sells, Arizona. The intent of this project is to provide a watertight roofing system that is maintainable and resists the transfer of heat. Ensure that the equipment is capable of maintaining proper temperatures and air volumes within the rooms. The equipment should be installed in such a manner to allow proper maintenance and serviceability. Carefully plan the work to minimize down time within the clinic and install the roof system and equipment in such a manner to avoid disruption to the occupants. Install energy efficient systems where possible. This project includes but is not necessarily limited to the following: (a) . Santa Rosa Clinic: 1) Tear off and replace existing roofing, including deteriorated wood decking. 2) Construct gabled roof on top of existing flat roof to provided drainage. 3) Remove and replace two existing air conditioning units, including new ductwork. 4) Remove and reinstall existing air conditioning unit including new ductwork. 5) Relocate existing vacuum pump and add new exhaust. 6) Remove and replace electric service to three air conditioning unites. 7) Remove and replace electric service to five exhaust fans. 8) Remove existing transfer switch. 9) Replace existing interior electrical distribution panel. (b.) Sells Transient Quarters: 1) Tear off and replace existing roofing, including deteriorated wood decking. 2) Remove and replace seven existing air conditioning units, including new ductwork 3) Remove and replace electric service to seven air conditioning units. The Contractor shall provide all labor, material, supplies, transportation, equipment supervision and all other necessary items for a complete and useable project, performing all work in a workman like manner in complete compliance with all plans, drawings, and specifications issued and approved. BID BONDS, PAYMENT AND PERFORMANCE BONDS ARE REQUIRED. NAICS CODES ARE 238160 ROOFING CONTRACTOR AND 23822 PLUMBING, HEATING AND AIR-CONDITIONING CONTRACTORS. SIZE STANDARD IS $27.5 Million for each. The estimated project size is between $ 100,000.00 and $500,000.00. SET-ASIDE NOTICE: This procurement is set aside under the Small Business Competitiveness Demonstration Program, FAR Part 19, as unrestricted competition. Additionally, in accordance with Public Law 100-656, this procurement is a 51% Buy Indian Set Aside. The proposed contract listed herein is Set Aside under the Buy Indian Act for 51 percent Indian owned, controlled and operated firms. In accordance with the Indian Health Service Acquisition Regulation, Paragraph 370.503(E), not more than 50 percent of the work to be performed under a prime contract awarded pursuant to the Buy Indian Act, shall be subcontracted to other than Indian firms. For this purpose, work to be performed does not include the provision of materials, supplies or equipment. Contractors shall indicate whether they are 51 percent Indian owned, controlled and operated firm. In order for Indian firms to prove they can meet the qualification requirements of the Buy-Indian Set Aside restriction, Offeror?s who are currently certified by the Indian Health Service or the Bureau of Indian Affairs as an Indian Firm, need to submit certification, Tribal Affiliation and Degree of Indian Blood with their offer. Offeror?s who are not currently certified by IHS or BIA are required to submit proof of ownership and eligibility for Indian preference under the Buy Indian Act. Joint Ventures will be considered as long as the Indian firm is the managing entity. A copy of the joint venture agreement must be submitted with offer. Technical Evaluation Factors/Past Performance and the Cost Price shall be furnished separately. MANDATORY REGISTRATION REQUIREMENT: FAR 52.204-7 Central Contractor Registration: Any and all firms or persons wishing to respond to this solicitation must be registered in the "Central Contractor Registration (CCR) database" which is the primary Government repository for Contractor information required for the conduct of business with the Government. Offerors and Contractors may obtain information on registration and annual confirmation requirements via the internet at http://www.ccr.gov or by calling 1-888-227-2423, or 269-961-5757. By submission of an offer, the offeror acknowledges the requirement that a prospective awardee shall be registered in the CCR database prior to award, during performance, and through final payment of any contract, Competitive formal Best Value Source Selection process in accordance with Part 15 of the Federal Acquisition Regulation (FAR) will be used to ensure selection of the source evidencing the best overall capability to perform the work in a manner most advantageous to the Government, as determined by evaluation of proposals in accordance with the established criteria. The qualifications submitted by each firm will be reviewed and evaluated to identify the most highly qualified firm. When responding to this announcement, firms should fully address their capability and qualifications with regard to each of the following evaluation factors. Firms should submit their complete qualifications as they may be evaluated solely on their written responses to this announcement. The top ranked firm will be evaluated and recommended solely on their written responses to the solicitation. Oral interviews may be held at the option of the Government. Prime Contractors assembling a team are cautioned that that the evaluation will be based on the team, not just the prime firm. The Offeror must demonstrate its experience and professional ability in the following areas: (1) EXPERIENCE AS A PRIME CONTRACTOR WITH PROJECTS OF SIMILAR SIZE, SCOPE, AND COMPLEXITY: Prime contractor past experience in specifically installation of roofing and HVAC replacement systems. Offeror must show construction experience on similar projects of similar size and scope. Elaborate on projects that have been completed in the last 3 years. Provide reference that may be contacted. (2) PROFESSIONAL QUALIFICATIONS AND EXPERIENCE OF THE PROPOSED STAFF FOR THIS PROJECT: Professional qualifications of project managers, superintendents, estimator, quality control, and construction inspectors. Technical competence and skills as they relate specifically to roofing and HVAC replacement projects. (3) MANAGEMENT PLAN: Proposed project approach, construction method to accomplish contract requirements Including teaming planning between contractor/sub-contractors, the designer of record and Government representatives. Cost management; Critical path scheduling; Value engineering; Fast track experience must also be presented for consideration. (4) QUALITY CONTROL/QUALITY ASSURANCE/ SAFETY EXPERIENCE DURING CONSTRUCTION PHASE: Description of specific safety and quality control initiatives and plans for this project and previous projects listed as past experience. (5) PAST PERFORMANCE WITH GOVERNMENT AND PRIVATE INDUSTRY CONTRACTS OF SIMILAR SIZE, SCOPE, AND COMPLEXITY: Past performance on contracts with Government entities including Native American organizations, tribal governments, federal agencies, i.e., Indian Health Service and Bureau of Indian Affairs. In accordance with FAR 15.305(a)(2)(ii), the Offerors is required to identify past or current contracts (including Federal, State, and local government and private enterprise) for efforts similar to the current Government requirements in this solicitation. The Government shall consider this information as well as information obtained from other sources when evaluating the Offerors past performance. The Offerors is reminded that the Contracting Officer shall determine the relevance of similar past performance information. The Offerors shall limit this information to not more than 10 contracts but no less than 3 similar contracts, performed within the last 3 years. Failure of an Offeror to provide a minimum of 5 relevant references or the inability of the Government to reach at least 3 references, after making a reasonable effort to do so, may result in the Offeror not being rated on the past performance factor. An Offerors who has no relevant experience will be given the mid-point rating of the maximum possible score allowed for past performance. Past Performance information must include the following information and be submitted in the following format and order: 1.: Contract Number; 2.: Contract Award Date; 3.: Contract Type (e.g., fixed price, cost reimbursable, etc.); 4. Contract Term (e.g., one year, base year and option years); 5. Contract Dollar Value; 6: Name of Procuring Organization; 7. Address of Procuring Organizations; 8. Procuring Organization Contracting Officer?s Name and Phone Number; 9. Procuring Organization Technical or Program Manager?s Name and Phone Number; 10. Indicate whether the contract was a prime contract or whether the contract was a subcontract to a prime contract. If a prime contract, insert ?Prime,? if a subcontract, insert subcontractor. 11. Provide information on problems encountered on the identified contract and the corrective actions taken. (6) PRICE EVALUATION FACTORS: Offerors are advised that an award may be made without discussion, exchanges or any contact concerning the proposals received; therefore, offerors should present their best and final price at the time of submission. Offerors should not assume that they will be contacted or afforded an opportunity to qualify, discuss, or revise their proposals. Proposals will be evaluated on lump sum price for the base item and evaluation of price on any options that is most advantageous to the Government. Award of any options items may be made by modification to the contract and, if exercised, would be awarded when funds become available during the term of the contract. The Government reserves the right to reject any or all proposals at any time prior to award; to negotiate with any or all offerors; to award a contract to other than the offeror submitting the lowest total price; and to award to the offeror submitting the proposal determined by the Government to be the most advantageous to the Government. The final selection will be determined based on both the technical proposal and the cost proposal submitted. Technical proposals addressing experience as a prime contractor on projects of similar size scope, professional qualification and experience of the proposed staff for the project, management plan, quality control/quality assurance/safety experience and past performance with Government and private industry of similar size, scope and complexity are to be addressed by each firm and are 60% of the award value. Price is 40% of Award Factor. A copy of the solicitation plans and drawings may be purchased for a non-refundable fee of $25.00 per set. A cashier check, bank or postal money orders must be made payable to the Indian Health Service and must accompany your request. CASH, COMPANY CHECKS OR PERSONNEL CHECKS WILL NOT BE ACCEPTED AND WILL BE RETURNED. Submit your written request along with payment to Chief of Contracting, Indian Health Service, Division of Engineering Services, 1301 Young Street, Suite 1071, Dallas, Texas 75202. A PRE-PROPOSAL CONFERENCE IS SCHEDULED FOR JANUARY 7, 2004 at 10:00AM. All interested parties should meet at the Sells Transient Quarters, Tohono O?Dham Indian Reservation, Sells, Arizona. An additional conference will be held at the Santa Rosa Transient Quarters at 1:30 PM on January 7, 2004. These are THE ONLY AUTHORIZED SITE VISIT. Qualification packages and price proposals are to be sent to: Mr. William E.
