Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotation s are being requested and a written solicitation will not be issued. The solicitation number, W9133N-04-T-0002 is being issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-18. The North American Industry Classification System Code is 561611 with a small business size standard of $10.5M. This acquisition will be issued as a 100% Small Business set-aside. The following provisions apply to thi s acquisition: 52.212-1, Instructions to Offerors-Commercial, Offerors should include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer, 52.212-4, Contract Terms and Conditions??????C ommercial Items. DFARS 252.204- 7004, Required Central Contractor Registration (CCR)(NOV 2001) applies. NOTE: CCR can be obtained by accessing the internet at http://www.ccr.gov or by calling 1-888-227-2423. The CCR number must be obtained prior to award being made. This will be a best value award and the Government may award without discussions. The Government will determine the proposal that provides the most advantageous offer to satisfy the requirements of this RFQ based on an integrated assessment o f each quotation. This selection will be based on the criteria below. The Government reserves the right to award to other than the contractor with the lowest price.The following criteria will be considered (in descending order of importance) 1.Technical capability, 2. Past Performance, 3. Price. The Idaho Air National Guard Safety Group requires the complete diagnostic analysis of the crash SDM of a 2001 Chevrolet Suburban vehicle involved in an accident that led to a fatality of an ANG member on 21 Octob er 2003, near Mt. Home AFB, ID. The purpose of this Statement of Objectives is to obtain an objective evaluation of the crash SDM through a complete diagnostic analysis to determine the vehicle??????s key data for the time period beginning thirty secon ds before the accident and ending thirty seconds after the accident. Deposition and/or trial expert testimony, with exhibit preparation is not expected at this time. The contractor shall provide all necessary labor, equipment, and services to complete th e following tasks: Provide written interpretation and fact set summary report with opinions, bases of opinions, document references,industry references, vehicle inspection data, and accumulated product knowledge (report). Maintain effective communications with the government program office assigned Safety Personnel. The contractor shall keep the respective assigned Safety Personnel informed of the current status of supported projects and tasks and inform the appropriate assigned Safety Personnel of any pro grammatic or logistics problems and/or resolutions of problems within two (2) working days of encountering such problem(s). The contractor shall be capable of explaining typical project/program management and logistics problems and solutions to the respec tive assigned Safety Personnel. Meet with the assigned Safety Personnel to ensure all data is complete and understood upon completion of data collection. If it is possible, download and compile information requested at Mt.Home AFB on the same day of colle ction. The period of performance shall be completed not later than fifteen(15) calendar days after contract award. The contractor shall complete a diagnostic analysis of the crash SDM. The analysis shall include, but not be limited to the following infor mation: Readiness of the airbag system; When the system commanded air bag inflation; The driver??????s safety belt usage at supposed deployment; Speed the vehic le was traveling thirty seconds prior to and after impact; Engine RPM??????s thirty seconds prior to and after impact; Brake status and throttle position thirty seconds prior to and after impact; State of driver??????s seat belt switch (On/Off); Passenger? ?????s air bag enabled or disabled state (On/Off); SIR Warning Lamp status (On/Off); Time from vehicle impact to air bag deployment; Ignition cycle count at event time; Ignition cycle count at investigation; Maximum DV for non-deployment event; DV vs. time for frontal air bag deployment event; Time from vehicle impact to time of maximum DV; Time between non-deploy and deploy event (if within 5 seconds); Any additional data that is retrievable that may aid in the investigation. The contractor shall be allowe d access to Government facilities during normal business hours 0700-1700, Monday through Friday, excluding Government holidays.The contractor shall coordinate any work hours outside of this period with the assigned Safety Personnel before work is scheduled . The contractor shall travel to Mt. Home AFB in Boise, Idaho. Space and computer equipment shall be provided for contractor personnel on Mt. Home AFB,ID. The Government shall furnish all typical office equipment to support this requirement. This equipme nt shall include a PC, desk, chair, phone, and access to copiers and the Internet. The Government shall provide the Contractor access to all required documents, specifications, and other applicable data upon notification. Contractor and subcontractor emplo yees shall be subjected to meeting conflict of interest standards IAW FAR 9.5 as appropriate for tasks resulting from this SOO. Such standards shall be approved and issued by the Contracting Officer. All data retrieved, and all reports concerning the data, shall be property of the United States Government. The contractor shall not publish or disclose such data or reports in any manner, without the written consent of the USAF Safety Center. All civilian personnel shall be US citizens and will be escorted wh ile on Mt. Home AFB, ID. The contractor shall provide a list of names and Social Security numbers, to base Security Forces for clearance. All contractor personnel must Submit proof of professional knowledge and expertise in forensic science, automotive i nvestigative techniques and data analysis. In addition, contractor personnel must submit proof of ability to retrieve and interpret the crash SDM. Any government furnished equipment and/or property given to the contractor must be returned in the same cond ition as when it was provided. The equipment and/or property must be returned immediately upon completion of the project. Deliverables: Written Interpretation and Fact Summary Report ?????? The report shall be delivered not later than fifteen calendar days after contract award. The contractor shall submit both an electronic (Microsoft Word) and hard copy of the report to the following address: National Guard Bureau, 1411 Jefferson-Davis Highway, Room 9400, Arlington, VA 22202-3231, Attn: CMsgt Don Williams . All work shall be in accordance with AFI 91-204.
