- REQUEST FOR INFORMATION ONLY: Statement of Requirements for DFAS-Wide Health Services. The Defense Finance and Accounting Service (DFAS) is seeking a health care provider to provide occupational health services in a clinical setting within a DFAS occupied facility or on demand for numerous DFAS locations (see attached list) within the 50 States. The agency requires basic services for employees at these locations. Also listed are premium services which may be requested if the agency desires. The initial RFI served to inform DFAS of service providers interested in providing these type services to DFAS. This request is to seek additional information regarding the pricing of these services. Request pricing estimates for the services listed below to include: 1. Basic Services as a ?package? of services 2. Basic Services priced as separately purchased services 3. Premium Services priced as separately purchased services Basic Services (Required): - Walk-in care/First Aid. Medical professionals provide first aid for minor injuries and illnesses and refer employees to primary care providers for follow-up care. - Emergency Response/AED Services. Staff will respond to medical emergencies and have state-of-the-art automated external defibrillators (AEDs) at each site to be used in cases of cardiac arrest. - Immunizations. Care providers provide a variety of immunizations, including those for influenza, tetanus, and pneumonia. Also would provide updated information for required and recommended vaccinations for official travel abroad. - Physician Prescribed Services. To minimize time away from work, the medical staff will administer services prescribed by the employee?s personal physician, such as allergy treatments and blood pressure and glucose monitoring. Premium Services (Optional): - Health Awareness Programs. Medical professionals will offer workshops on topics such as cancer risks, nutrition, smoking cessation, and stress reduction. These programs, in combination with health education materials, empower employees to make lifestyle changes that improve their health and well being. - Health Screenings. Clinic would offer a wide variety of health screenings including high blood pressure, cholesterol, diabetes, colon cancer, tuberculosis, and vision. - Recordkeeping. Clinic maintains employee health records and ensures compliance with all applicable provisions of the Privacy Act. - Emergency Planning. Clinic staff would participate in emergency planning, so that DFAS sites are properly prepared for fires, bomb threats, and natural disasters. - Health Risk Appraisals. Service provider would have available, either on-line or in paper form, a health risk appraisal to give employees a personalized analysis of their health risks. From that appraisal they would provide specific recommendations about lifestyle changes that can help decrease their risk of cancer, heart attack, or stroke ? the leading causes of death and disability in the United States. - Employee Paid Services. In addition to health screenings performed at no cost, the service provider will arrange access to other on-site medical screenings whereby the fees are paid by or billed directly to the employee, providing important health services at a convenient location without increasing cost to DFAS. With DFAS approval, screenings may include mammograms for breast cancer, bone density for osteoporosis, electrocardiograms and various laboratory testing. - Management Reports. Service provider would supply reports about utilization of services, as well as a summary of health risk appraisals conducted during the year. These reports would be used by DFAS to give a picture of overall health of the workforce and are essential for measuring improvements and program planning. - Automatic External Defibrillator (AED) Program: Provide medical director oversight, customized program for various sites, site-specific protocols, CPR and AED training, sudden cardiac arrest event analysis, incident stress management, and review of existing program(s). - Smoking Cessation Program: Have the ability to provide a customized smoking cessation program to meet agency?s needs. Various options should include but not limited to nicotine replacement therapy, self-directed cessation resources, and group programs. - Ergonomic Services: Provide a range of services from ergonomic specialists offering program development, on-site interactive training, individual and group ergonomic workstation assessments, workplace design consultations, and accommodation consultations.
