Description: The Aberdeen Area Indian Health Service is currently seeking potential bidders for the purpose of a non personal, firm-fixed price contract for the provision of Physical Therapy services for both in-patient and out-patient, located at the PHS Indian Hospital, Pine Ridge, South Dakota 57770. This will be for the period of one year. The potential contractor will work eight (8) hours a day/five days per week. This contract will be set-aside for small business only. To obtain a copy of this RFQ, please submit in writing via fax or e-mail at (605)226-7669. The last date to request a copy of the RFQ will be December 12, 2003.
