- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Provide an office assistant for the Exceptional Family Member Program (EFMP), Ft. Belvoir, VA. All work shall be performed in accordance with the Statement of Work. Solicitation number W91QV1-04-T-0011 applies and is issued as a Request for Quotation (RFQ) . This solicitation document, incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-18. This procurement is 100% set aside for small business. The NAICS code is 561110 and the small business size standard is $6.0 M. Any supplies and services to be furnished under this requirement shall be executed by the issuance of a purchase order. This requirement is for a fixed price contract with performance period of base year and two (2) options years that will be exercised at the option of the Government: CLIN 0001, Base Year: 1 Feb 2004 - 31 Jan 2005; The Exceptional Family Member Program (EFMP) shall acquire an office assistant. All work shall be performed in accordance with the attached Statement of Work (SOW): CLIN 1001, Option Year 1: 1 Feb 2005 ?????? 31 Jan 2006; The Exceptional Family Member Program (EFMP) shall acquire an office assistant. All work shall be performed in accordance with the attached Statement of Work (SOW): CLIN 2001, Option Year 2: 1 Feb 2006 ?????? 31 Jan 2007; The Exceptional Family Member Program (EFMP) shall acquire an office assistant. All work shall be performed in accordance with the attached Statement of Work (SOW). Provision at FAR 52.212-1 Instructions to Offerors-Commercial Items applies. P rovision at FAR 52.212-2 Evaluation-Commercial Items applies with paragraph (a) as follows; The following factor shall be used to evaluate offers: Personnel, Experience, and Price. Proposals that are deemed ??????technically acceptable?????? in accordanc e with Addendum FAR 52.212-2, Evaluation ?????? Commercial Items, will advance to price evaluation as described in ??????3?????? of the aforementioned Addendum. Award will be made to the lowest priced, technically acceptable proposal. The Contracting Officer may exercise the opt ion by written notice to the Contractor within 30 days.; 52.217-9 Option to Extend the Term of the Contract (a) The Government may extend the term of this contract by written notice to the Contractor within 60 days; provided that the Government gives the C ontractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension, and (c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 3 years.; 52.232-19 Availabil ity of Funds for the Next Fiscal Year. STATEMENT OF WORK Exceptional Family Member Program Office Assistant C.1 GENERAL C.1.1 The purpose of the Exceptional Family Member Program (EFMP) is to assist military families who have special needs family members by providing information, assistance, and other supportive services to minimize stress and problems related to accomm odating special needs family members. C.1.2 SCOPE OF WORK C.1.2.1 The EFMP contractor (hereafter referred to as ??????contractor??????) works independently within the Soldier and Family Support Center ?????? Army Community Services, Exceptional Family Member Program. The contractor shall accomplish the followi ng objectives: C.1.2.1.1 The Contractor shall have the responsibility for completing various office related tasks to include but not limited to: a. filing client records b. creating and maintaining client files c. typing memorandums d. answering and returning phone calls e. disseminating information (to include brochures and welcome letters) f. preparing reports g. coordinating the quarterly installation EFMP meeting h. writing the minutes for the meeting i. typing up the minutes to be signed by the Director of Community Activities (DCA) j. copying k. faxing l. maintaining the EFMP resource library m. updating and maintaining the EFMP civilian and military resource guide (Fort Belvoir Area) n. publishing monthly event calendars for the EFMP C.1.2.1.2 The Contractor shall have the responsibility for maintaining a weekly/monthly Parent Support Group by completing the following requirements: a. coordinating appropriate meeting areas b. preparing necessary materials; c. coordinating and publishing a calendar of meeting dates, times, and places c. coordinating and publishing a calendar of meeting dates, times, and places d. preparing and forwarding monthly meeting statistics to the EFMP manager e. providing information related to civilian and military community resources to parents requesting referrals C.1.2.1.3 The Contractor shall have the responsibility for maintaining a monthly Family Arts and Crafts project by completing the following requirements: a. coordinating appropriate meeting areas b. preparing necessary materials c. coordinating and publishing a calendar of meeting dates, times, and places d. preparing and forwarding monthly meeting statistics to the EFMP manager e. providing information related to civilian and military community resources to parents requesting referrals C.1.3 Operating Hours C.1.3.1 The Contractor shall work onsite at Soldier and Family Support Center; 9655 Belvoir Road, Building 1001; Fort Belvoir, VA, 20 hours per week. C.1.3.2 The Contractor shall work the following schedule: Monday: 0930-1500 hours Wednesday: 0930-1500 hours Friday: 0930-1500 hours Saturday: 0930-1500 hours C.1.3.3 The EFMP manager or alternate shall be able to change work hours to meet office requirements with 48 hours notice; i.e., the manager may substitute weekday work hours to be completed on Tuesdays and Thursdays as needed. The Contractor will not st art work before 0800 or end work after 1630, if the work schedule is changed. C.1.3.4 There is a 30-minute lunch built into the schedule. C. 1.3.5 Holiday Pay. The Contractor shall not be paid for any federal, state or religious holidays. The Contractor may come in on an alternate day to make up work missed on federal holidays. C.1.4 PERSONNEL C.1.4.1 The Contractor shall have a minimum of 5 years experience working or volunteering in Human Resources, specifically with Army Community Service. The Contractor shall also have at least 6 months experience in Exceptional Family Member Program. C.1.4.2 The Contractor shall have at least 100 credit hours of college that includes social work and psychology. C.1.4.3 The Contractor shall be skilled in using Microsoft Office Software. C.1.4.4 The Contractor shall be skilled in communicating effectively with professionals, military and civilian personnel, family members and groups. C.1.4.5 The Contractor shall assign only one individual to this task. Should this individual leave; the replacement must be approved by the EFMP manager/alternate and must meet qualifications set forth in Sections C.1.4.1 through C.1.4.4. C.1.5 GOVERNMENT SUPPLY ITEMS AND PROPERTY C.1.5.1 The Government property, facilities, equipment made available under this contract shall be used only in performance of this contract. C.1.5.2 The Government will not be responsible in any way for damage to the contractor??????s supplies, materials, or personal belongings brought into the building due to fire, theft, accident or other disaster. C.1.5.3 The Contractor shall be responsible for receiving, caring and accounting for any Government furnished property. C.1.5.4 The Government shall provide workspace for the contractor. C.1.6 CONFIDENTIALITY C.1.6.1 The contractor shall maintain strict confidentiality regarding personal information of clients. Information may be released to other military agencies pertaining to special problems, with the specific consent or request of the client. C.1.8 OTHER C.1.8.1 The EFMP manager reserves the right to review any and all resumes submitted by the prospective contractors to ensure the prospective contractors have met the required experience. ADDENDUM FAR 52.212-1, INSTRUCTIONS TO OFFERORS The Government contemplates award of a Firm Fixed Price contract with a base year and two one-year options, if exercised. The period of performance for the base year shall be 1 February 2004 through 31 January 2005. Technical Proposal. The technical proposal shall include the following: a. Personnel. Evidence of the appropriate college required applicable to this requirement. b. Experience. Demonstrate experience as defined in Clause 52.212-2, Addendum. Price Proposal. a. SF 1449 cover sheet appropriately filled out. b. Solicitation Schedule (page 2 of the solicitation) completed. All questions shall be submitted in writing to Ellen_B_Harden@belvoir.army.mil ADDENDUM 52.212-2 EVALUATION ?????? COMMERCIAL ITEMS The Government intends to select the lowest priced, technically acceptable proposal submitted by a responsible offeror. The Government intends to award without discussions. Accordingly, offerors are advised to submit proposals that fully satisfy the Govern ment??????s minimum requirements defined in CLINS 0001 through 2001 and that include the offeror??????s best price. Technical Acceptability Determination 1. Proposals will be evaluated for technical acceptability according to two (2) separate evaluation factors: Personnel and Experience. A determination, after a final technical acceptability evaluation, that a proposal is rated lower than ??????acceptable?? ???? (for any factor) will disqualify the proposal from further consideration. a. Personnel. Must have a minimum of 100 credit hours of college that includes social work and psychology. b. Experience. Must demonstrate a minimum of five (5) years experience in Human Resources with Army Community Service. Must have minimum of six (6) months experience in Exceptional Family Member Program. Can didates must have a working knowledge of Microsoft Office Software. Also be skilled in communicating effectively with professionals, military and civilian personnel, and family members and groups. 2. For purposes of the technical acceptability determination, the following definitions will apply: a. Acceptable. A proposal is acceptable for purposes of a technical evaluation factor if it meets or exceeds all requirements of the solicitation and the pre-established standards for every area and factor being evaluated. b. Unacceptable. A proposal is unacceptable for purposes of any technical evaluation factor if it fails to meet one or more of the pre-established standards for any area and factor being evaluated. 3. Following the technical evaluation, the Government will review all proposals deemed technically acceptable and will determine which one offers the lowest proposed price. Wage Determination #94-2103 REV-30, dated 06-03-2003 applies to this requirement. Responses to this combined synopsis/solicitation must be received on 7 January 2004 by 1:00PM Eastern Standard Time.
