Description
The Air Armament Center (AAC), Eglin AFB, FL intends to award a Firm-Fixed Price contract for a Non-Personal Relationship Counseling Service. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number F796CC3300100 is issued as a Request for Quote (RFQ) using Simplified Acquisition Procedures found in FAR Part 13. The North American Industry Classification System (NAICS) code for this acquisition is 624190, with a small business standard size of $6,000,000. This is a 100% Small Business Set-Aside. The solicitation will result in a single award. The period of performance is for a base period from February 1, 2004 thru September 30, 2004, with (4) one(1)-year options. Interested contractors must provide (1) qualified Therapist licensed to practice in the state of Florida with required qualifications: Licensed Clinical Social Worker(LCSW), Licensed Mental Health Counselor(LMHC), Licensed Marriage & Family Therapist(LMFT), or Licensed Psychologist. Additional requirements are: Master?s Degree in Social Work, Marriage and Family therapy, Mental Health, other Behavioral Sciences discipline or a Ph.D. in Psychology from accredited schools 2) Minimum of five years full time, post- license experience in a professional clinical counseling. Related experience will not be accepted in lieu of degree or license requirements. Minimum of three years experience of individual, marital/partner, or family therapy in private practice or in a social service agency (3) Proficiency in the English language both written and oral, and 4) price. To be eligible for award, individuals must have current or be able to pass the Federal and state criminal history background checks. Direct relationship counseling service is typified as: 1) provide intake screening and assessment; 2) provide psycho-educational service to identify readjustment difficulties, education, positive growth and developmental counseling service on relationship and parenting issues; 3) coordination and referrals to other base helping organizations, and 4) promote relationship counseling service to Eglin AFB military community to encourage voluntary usage of the service among active duty members and their spouses. The Therapist will provide a six-session curriculum. The contractor will submit qualifications packages (curriculum vitae) for only those contract Therapist who can reasonably be anticipated to render the actual and substantial performance of the contract. Three professional references are required within the past year, from sources that are knowledgeable of the Therapist?s clinical interaction. The submission of primary contract Therapist who lack the requisite qualifications will be considered nonresponsive. Performance of service will be at the Chaplain?s office, Eglin AFB Florida, on 1 February 2004, FOB Destination. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-10. Evaluation will be based on the following factors: (1) technical (licensure, education, and other professional credentials) (2) quality and relevance of experience; (3) three professional references within one year, (4) proficiency in the English language both written and oral, and (5) price. The following factors: factor 1) technical and factor 2) experience are significantly equal in importance and significantly more important than price.
