- This is a combined sybnopsis/solicitation. Request for Proposal #RFP 603-7-04 is hereby issued for commercial items prepared in accordance with the format in FAR Part 12, Acquisition of Commercial Items, Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, Part 13, Simplified Acquisition Procedures, and as supplemented with additional information included in this notice. This announcement incorporates provisions and clauses in effect through FAR FAC 2001-17 and VAAR 97-8. NAICS code 811219 applies with small business size standard of $6Mil. This announcement constitues the only solicitation and a written solicitation will NOT be issued. All proposals/quotes must meet the listed requirements in this announcement. A response to requirements listed below shall be submitted on contractor's letterhead or formal quote form and must address all requirements listed in a separate technical proposal. Contractors price and technical proposal must contain the name; title and signature of person authroized to submit proposals/bids on behalf of the contractor. Descriptive literature for facility must also be included in proposal/bid package(price and technical must be seperated). The VAMC Louisville Ky has identified a need for preventive maintenance and repairs on the PICKER MRI, model EDGE,and related accessories including magnet cooler chiller, two condensers, to include the magnet and cryogens. Base period is Feb 1, 2004 through September 30, 2004 with 4 one-year renewal options, Oct 1, 2004 through September 30, 2008. Proposal shall include all information required in 52.212-1, Schedule of Prices Offered, a copy of published price list for the services proposed, and a completed copy of provision 52.212-3. Evaluation of proposal offered will include the following factors listed in decending order of importance: past performance history (include threee corp or govt references with telephone numbers; capability to meet the Govt's requirements; and price. Offeror shall submit a price for Monthly Preventive Maintenance (normal working hours 8AM - 5PM)and an hourly price for Emergency maint/repairs after normal hours. Submit all price, descriptive literature and technical proposals to Victoria Caudill (90C), VAMC, 800 Zorn Ave., Louisville, KY 40206-1499. Offerors proposals are due by 3:30Pm local time on January 20, 2004. Faxed proposals will NOT Be accepted. The VA intends to award a firm fixed price contract to the offeror determined to be the best valaue to the Government. evaluation may be made with or without discussions with the offero(s). ALl responsible sources that can meet the requirements as specified in this notice may apply.
