- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation RFQ 244-04-00123 is being issued as a Request for Quotation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-18. This procurement is unrestricted. NAICS Code 333312, 500 employees small business size standard applies. Items will be purchased and delivered to the VA Pittsburgh Healthcare System, 7180 Highland Drive, Pittsburgh PA 15206. Prices shall be submitted to furnish and install the following commercial laundry equipment and related parts/services: 1) TWO (2)-FOLDER CROSS FOLDER WITH STACKER AND CONVEYOR, Less Trade-in allowance "AS IS - WHERE IS" condition for 1 ? each Braun Folder Cross Folder ? Model 4PF3CFOMEGA ? Serial Number ? OM85430746; 1 ? each Braun Large Piece Stacker and Conveyor ? Model ST1 ? Serial Number ? 985264; 1 ? each Braun Folder Cross Folder ? Model 2PF3CFOMEGA ? Serial Number ? OM85230745; 1 ? each Braun Large Piece Stacker and Conveyor ? Model ST1 ? Serial Number ? 985265. 2) TWO (2) SMALL PIECE FOLDING MACHINE Less Trade-in allowance "AS IS - WHERE IS" condition for 1 ? each Braun, Model Number SIGMA, Serial Number ? SP985544, 1 ? each Braun, Model Number SIGMA, Serial Number ? SP985265, removal of trade-in shall be the contractor?s responsibility. 3) TRAINING MATERIALS AND VIDEOS: Contractor shall submit training materials and training videos (prepared in VHS, CD or DVD format) on each piece of equipment for operation and maintenance. Separate training videos shall be made for operation and maintenance and shall not be combined. These training materials and videos shall demonstrate the operation and maintenance of the equipment and any adjustments or other actions that may be undertaken by VA operating personnel in the event of failure of equipment. These training materials and videos shall be submitted to the responsible operations official or his designee prior to beginning of training. Solicitation documents have been uploaded as an attachment to this synopsis/solicitation. Offerors wishing to view the VA Pittsburgh Healthcare System laundry prior to submitting their quote may do so by contacting Mr. Leroy Myers, Textile Care Manager at 412-365-5980. The following provisions and clauses apply to this solicitation: 52.212-1 Instructions to Offeror-Commercial Items, 52.212-4 Contract Terms and Conditions-Commercial Items, 52.212-5 Contract Terms and Conditions Required to Implement Statues or Executive Order Commercial Items. FAR 52.233-2 Service of Protest, VAAR 852.233-70 Protest Content, VAAR 852.233-71 Alternative Protest Procedure and VAAR 852.270-4 Commerical Advertising also apply to this solicitation. Quotes will be evaluated on the following criteria: technical capability of equipment, price and service agreement offered. Award will be made to the offeror whose products meet the requirements listed above and is most advantageous to the Government. The Government intends to evaluate the information received and award without discussions. The Government reserves the right to make awards on the above equipment and related supplies/services. Offers including the pricing information are to be submitted and include the information required in provisions 52.212-1, a completed copy of the provisions 52.212-3 Offeror Representations and Certifications-Commercial Items, literature/brochures on proposed equipment. The Offeror shall also provide its Commercial and Government Entity (CAGE) code, Contractor Establishment Code (DUNS number) and Tax Identification number.. Electronic copies of the referenced provisions and clauses are available on the Internet at www.arnet.gov/far and www.va.gov/oamm/vaar/. Original and 1 copy of quotation and all required information must be received by 2:00 PM local time on January 30, 2004. Quotations shall be submitted to Kathleen M Grundza, Contracting Officer, VA Pittsburgh Healthcare System. Faxed quotations will not be accepted. Solicitation number must be listed on all documents for identification purposes. Only offers prepared in accordance with this notice will be considered.
