- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulations document (FAR) subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number FA4600-04-T-0013 is being issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-18. All applicable provision and clauses are included in the actual solicitation. This electronic announcement constitutes the only solicitation being issued. This procurement is set aside 100% for small business. The North American Industry Classification System (NAICS) code is 713940, and the small business size standard is $6.0M. The Government intends to issue a firm fixed price purchase contract for the following: Provide water aerobics classes IAW the statement of work: WATER AEROBICS 1. The Contractor must work within the established guidelines of the Air Force Instructions, Operating Instructions, rules, policies, etc., which are governed by Outdoor Recreation, AFI 34-110, AFMAN 34-233 and SVR-01 34-113 and Offutt AFB Lifeguard Policies and Procedures Manual. 2. The Contractor will act as the Water Aerobics Instructor at the Offutt house Indoor Pool, and requires a CAC/AF Form 75 for access on Offutt AFB. 3. The contractor should be responsible for providing a variety of classes and scheduling water aerobic instructors for 9 classes per week. Classes shall be one hour in length. The contractor develops what will be taught. Classes are scheduled Monday through Friday 8:30 ? 9:30 and Monday through Thursday 17:30 ? 18:30. Any rescheduling will be coordinated between the Program Director and the Contractor. The responsibility of reserving the pool, advertisement, rescheduling, and informing all patrons of the changes belongs to the Program Director and the Pool Manager. 4. Contractor?s responsibilities include, but are not limited to, the following: a. The contractor will provide the Aquatics Director a copy of his/her CPR certification and any nationally recognized certificate to instruct water aerobics. Copies of the CPR and any other certifications will be maintained by the activity (Outdoor Recreation) and the Contracting Office. b. The contractor must provide proof of liability insurance. Copies will be maintained by Outdoor Recreation and Contracting. c. Instruction of all Class participants will be conducted in a safe manner, a qualified lifeguard will be on duty during all scheduled classes. d. The contractor is responsible for arriving at the Indoor Pool 10 minutes prior to the beginning of the class and starting all classes (LESSONS) on time. e. Responsible for the set up and take down of all items used or moved for the water aerobic classes. The government is providing the below listed equipment (contractor moves them): Ropes, audio equipment (including speakers, CD player, wireless water aerobics microphone), water weights, and weight carts. The contractor selects and provides the music. f. The Contractor is required to notify his/her students one hour in advance if classes are cancelled. g. The Program Director will determine and approve class schedule prior to award of contract. h. Make-up days for cancelled classes will be coordinated with the Pool Manager and/or Program Director. All make-up classes should be schedule within the next week. A doubleheader (scheduling a make-up class in coordination with the class) will depend on other activities scheduled at the pool. The period of performance is CLIN 001: Base Year (1 JAN 04 to 31 DEC 04); CLIN 0002: Option year one (1 JAN 05 to 31 DEC 05); CLIN 0003: Option year two (1 JAN 06 to 31 DEC 06; CLIN 0003: Option year three (1 JAN 07 to 31 JAN 07); CLIN 0004: Option year four (1 JAN 08 to 31 DEC 08). As long as the government provides the contractor with preliminary written notice at least 30 days prior to contract expiration, the government may extend the length of the contract 15 days prior to expiration. This contract, including options, shall not exceed 66 months.
