Description
This is a combined synopsis and solicitation for the following Service: Contractor shall provide an on-site mock accreditation survey and conduct a Joint Commission on Accreditation of Health Care Organization (JCAHO) education program. Contractor shall provide as 2-day mock-survey and 1 day of education for a total of three days. Quotes should include but are not limited to the following: transportation to Bitburg AB and Spangdahlem AB, Germany, lodging, car rental (if applicable) and per diem. Quotes should be in accordance with the following statement of work: GENERAL: This requirement is two-fold: (1) Provide leadership education and development related to the ?2004 Shared Vision, New Pathways? Joint Commission on Accreditation of Healthcare Organizations (JCAHO) standards/ (2) Using the new ?tracer methodology?, perform a modified mock JCAHO review to identify potential JCAHO standard deficiencies and to develop a plan to correct the deficiencies. SCOPE OF WORK: For the first requirement, the contractor shall provide one facilitator for one day to conduct education and training to Executive Leadership and key leaders from each squadron on JCAHO standards and preparation using the 2004 Shared Vision, New Pathways criteria. Upon completion of the training, the participants will be able to 1) List the major grouped performance based standard, 2) Describe ways they reflect the standards in their day-to-day work, 3) List documents required by JCAHO to demonstrate compliance and 4) List as least one strategy, related to their role, to be continuously ready to show compliance to the standards related. For the second requirement, the contractor shall provide two surveyors for two days to perform the modified mock review using the new ?tracer methodology? and JCAHO standards, and develop a plan to correct deficiencies. The contractor shall provide feedback during the mock survey and a summary at the end of the survey. Specifically, the contractor will include review of Medical Group Instruction and Management Plans for facilities at the 52nd Medical Group, Spangdahlem Air Base, Germany also referred to as the ?Medical Treatment Facility? (MTF) herein. Performance shall be according to the requirements contained in this performance work statement (PWS). The contractor shall provide all labor, tools, material, equipment and transportation necessary to conduct a JCAHO education program. PERSONNEL: For the first requirement, the contractor shall provide one facilitator for one training day to executive leadership and key squadron leaders of the MTF?s choosing. For the second requirement, the contractor shall provide two surveyors for two days to perform the modified mock review. POINT OF CONTACT: The contractor shall provide a point of contact that shall be responsible for the performance of the work. The point of contact may be an individual providing services in accordance with this PWS. The contractor shall designate this individual, in writing, to the Contracting Officer (CO) before the contract start date. An alternate may be designated, but the contractor shall identify those times when the alternate shall be the primary point of contact. AVAILABILITY: The contractor shall provide training during MTF duty hours, 0730-1630. ENGLISH LANGUAGE REQUIREMENTS: On site personnel shall read, understand, speak and write English. EMPLOYEES: The contractor shall not employ persons for work on this contract if such employee is identified to the contractor by the CO as a potential threat to the health, safety, security, general well being or operational mission of the installation and its population. Contract personnel shall present a neat appearance and be easily recognized as a contract employee. Access to Spangdahlem AB will require a temporary vehicle pass for which the MTF will arrange. Contract personnel shall wear seatbelts while on base and abide by all regulations and laws related to Spangdahlem AB and its federal properties. EDUCATION AND TRAINING REQUIREMENTS: Contractor personnel shall have extensive operational experience in all health care setting, specifically in military treatment facilities. Contractor personnel shall have hands-on experience in JCAHO standards and survey process development activities. Contractor personnel shall present evidence of previous military healthcare operation experience with respect to JCAHO preparation via education or mock surveys. CERTIFICATION: Contract personnel will have one or more of the following credentials: Certified Professional Healthcare Quality; Master?s Degree in Health Services Management, Healthcare Administration, Nursing or equivalent degree with emphasis on both healthcare operations and business processes; be a physician; or have experience as a former JCAHO Surveyor. SOFTWARE PORFICIENCY: Contractor personnel shall be proficient as required to conduct professional JCAHO education and training seminar. VALIDATION OF CURRENT CERTIFICATION: Within seven (7) days after contract award, the contractor shall provide the Quality Services Manager evidence of previous military healthcare operation experience with respect to JCAHO preparation, education, and/or mock surveys. Contractor shall also provide verification of specific credentials of the facilitators prior to the individual furnishing services in accordance with the contract. GOVERNMENT APPROVAL TO PROVIDE SERVICES: Approval from the MTF Commander is required before a facilitator may be allowed to conduct training. HOURS OF OPERATION: Contract personnel will be available on site (in the MTF) from 7:30 A.M to 4:30 P.M. on the training day with a sixty (60) minute lunch period and two fifteen (15) minute breaks per day. LIABILITY RESPONSIBILTY: The contractor shall provide services as an independent contractor. As such, the facilitator shall not be afforded coverage under the Federal Tort Claims Act as provided in the Medical Malpractice Immunity Act, 10 U.S.C. 1089. EMERGENCY HEALTH CARE: The MTF will provide emergency health care for contract personnel for injuries occurring while on duty in the MTF. These services will be billed to the contractor at the current reimbursement rate. COMPLIANCE WITH HEALTH INSURANCE PORTABILITY AND ACCOUNTABILITY (HIPPA): The contractor agrees to adhere to the requirements and restrictions of the Privacy Act of 1974 and the law as it relates to HIPPA. In the likely event that contract personnel are exposed to patient information, they will make reasonable attempt to protect patient information from further disclosure in accordance with HIPPA. CONTRACTING OFFICER (CO): A person duly appointed with the authority to enter into, administer, and terminate contracts on behalf of the Government. MILITARY TREATMENT FACILITY: An organized group of people, with the associated facilities and equipment, who provide medical services for a beneficiary population. GOVERNMENT FURNISHED PROPERTY AND SERVICES: The Government will provide the room with chairs for which to conduct the JCAHO training. The mock interview will be conducted in the clinics and inpatient unit. Any other meetings and interviews will be arranged with a room. FACILITIES: Contract personnel shall have the use of the facilities at the 52 MDG necessary for performing all services required by this Statement of Work. Facility use will be for official use only IAW applicable DoD and Air Force Instruction. CONTRACTOR FURNISHED ITEMS AND SERVICES: Except for those items or services specifically stated above to be furnished y the Government, the contractor shall furnish everything required to perform this contract in accordance with all of its terms. Specifically, the contractor shall provide all labor, tools, materials, equipment, and transportation necessary to conduct the training and inspection. This synopsis and solicitation is prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. North American Industrial Classification Standard 611430 applies to this procurement. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quote (RFQ). Submit written offers (oral offers will not be accepted), on RFQ F53SGL33500100. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-14 and Defense Acquisition Circular 91-13. FOB Destination: Inspection to take place at both Spangdahlem AB and Bitburg AB, Germany. The following provisions and clauses apply to this acquisition: FAR 52.212-1 Instructions to Offerors- Commercial Items, FAR 52.212-2 Evaluation-Commercial Items, FAR 52.212-3 Offerors Representations and Certifications- Commercial Items, FAR 52.212-4 Contract Terms and Conditions-Commercial Items and 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders- Commercial Items.
