- TITLE: Ambulance Services SOL: RFP10N3-72-04 DUE: 02/17/04 NAICS: 621910 AGENCY: Attn: Department of Veterans Affairs Medical Center, 79 Middleville Rd., Northport, NY 11768. POC: Contracting Officer ? Karen Diehl, Contract Specialist, (631-) 261-4400 ext. 7130 DESCRIPTION: The Northport VA Medical Center, 79 Middleville, Rd. Northport, NY 11768 has a requirement for 24 hour per day, 7 days per week advanced life support and basic life support Ambulance Services for beneficiaries of the Northport VA Medical Center, will primarily be provided in New York (Suffolk and Nassau Counties). All offerors must be licensed for operation in those areas. Only offers from those firms regularly established in the business of providing ambulance service will be considered. Period of performance will be from will be from the date of award through September 30, 2004 with provisions for two (2) one year options to extend. This is a Firm-Fixed price contract. The Government reserves the right to award this contract on another than lowest price, based upon a technical evaluation, which will be assessed prior to award. NAICS code 621910 applies. Electronic bids will not be accepted. Offerors are advised that they are responsible for obtaining amendments.
