Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a combined synopsis/solicitation for commercial service. This announcement constitutes the only solicitation. The California National Guard has an immediate requirement for a mo bile dentist at Camp Roberts, CA for troop mobilization. GOVT IS UNABLE TO PROVIDE BUILDING SPACE TO THE CONTRACTOR. REQUIREMENTS: *Estimated 160-200 bitewing x-rays *Provide dental treatment (fillings & extractions only) on estimated 25-35 soldiers. LOCATION: Camp Roberts Military Reservation (10 miles north of Paso Robles, CA) adjacent to Bldg 307. PERIOD OF PERFORMANCE: 23-27 January 2004 with possible extensions if warranted. GOV??????T RESPONSIBILITIES: *Parking access & secure location for Contractor, equipment & vehicles *Land-based power *Portable water supply *Available restroom facilities at nearby location *Nat??????l Guard will select soldiers to be seen. Once soldiers are identified, dental manager will schedule appropriately for a m aximized schedule *Nat??????l Guard will be responsible for reviewing recommended treatment plans with the lead dentist associated with Contractor *Map, directions and any access cards necessary for Contractor & staff. CONTRACTOR RESPONSIBILITIES: Delive ry & set-up of mobile dental practice *Staff, dental supplies & dental resources *Functioning dental opertories, lab and reception area within mobile dental practice. This synopsis is 100% set aside for small business. Simplified Acquisition Procedures wi ll be utilized. Award will be based on price and past performance. All vendors must be registered in the Central Contractor Registration (CCR).
