- Description
- The GLAC (Great Lakes Acquisition Center) is soliciting quotations to provide Pharmacy Services to eligible veteran beneficiaries of the Department of Veterans Affairs. Service shall include filling and dispensing emergency prescriptions and controlled substances in quantities up to ten day supply for prescriptions written by the care providers employed at the Union Grove Community Based Outpatient Clinic (CBOC), located in Union Grove, WI. Contract Period of Performance is April 1, 2004 through March 31, 2004, with two (2) one year option renewal periods. Option periods are exercised at the sole discretion of the Government. Contract type is firm-fixed price. All qualified offerors/firms who believe they can satisfy this requirement are invited and encouraged to provide their proposal. Offers, including options, shall be evaluated for fair and reasonable pricing, technical competency/capability to perform services and satisfactory past performance record. The Government anticipates award of a commercial item contract to the responsible offeror whose offer provides the best value to the Government. Award will be made utilizing the Simplified Acquisitions Procedures outlined in FAR subpart 13.5, and a combination of FAR part 12, 13, 15 & 17 award procedures. Anticipated award effective date is March 19, 2004. This is a Firm Fixed Price contract and this acquisition is a 100% small business set-aside. The size standard for North American Industry Classification System (NAICS) code 446110 is $6.0 Million. Offerors are reminded to complete all sections of the solicitation, including the Offeror Representations and Certifications-Commercial Items.
