- THIS COMBINED SYNOPSIS/SOLICITATION IS CANCELLED IN ITS ENTIRETY. COMBINED SYNOPSIS/SOLICITATION. AEROBICS SERVICES PROVIDER. (i). This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii). This solicitation is issued as a request for quote (RFQ). Submit written quotes (oral quotes will not be accepted) on RFQ number F7SVMP40130300. (iii). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-16. (iv). This procurement is being issued as a 100% small business set-aside. The North American Industry Classification System (NAICS) code is 611620 with a small business size standard of $6.0M. (v). This combined synopsis/solicitation is for the following commercial item: aerobics services provider. Requirements: Furnish instruction for aerobics training by teaching classes in various types of aerobics activities in accordance with the Statement of Work. An average of 20 classes per week will be taught at the Sports and Fitness Center at Malmstrom AFB Montana to authorized patrons. The requirement shall consist of: safe and quality instruction to all participants according to Air Force Instruction 10-248 (Fitness Program) and the Golden Eagle Standards for Services in the Air Force. Instructors must read, understand, speak and write English. The aerobics services provider will provide all music, as well as microphones and headsets for all classes. Certification requirements: instructors must have a current Cardiopulmonary Resuscitation certification and knowledge of first aid and emergency procedures. Instructors must have a current instructor?s certification from the Aerobic and Fitness Association of America, International Dance and Exercise Association, REEBOK, YMCA, SCHWINN, Cooper?s Institute or other nationally recognized certification organization. Aerobics instruction will be geared to strengthen cardiovascular endurance, decrease body fat, and improve lung capacity. (vi). FOB Destination ? Sports and Fitness Center, Building 1010, 7547 Goddard Drive, Malmstrom Air Force Base, Montana 59402. (vii). The Government will issue a blanket purchase agreement (BPA) resulting from this solicitation to the responsible party providing a quote, whose quote conforming to the solicitation will be most advantageous to the Government, price and other factors considered. (viii). The provision at FAR 52.212-1, Instructions to Offerors Commercial Items, applies to this solicitation. (ix). The provision at FAR 52.212-3, Offeror Representations and Certifications Commercial Items, applies to this solicitation. The party providing a quote must include a completed copy of this provision with its quote. A copy of this provision is located at http://farsite.hill.af.mil/. (x). The clause at FAR 52.212-4, Contract Terms and Conditions Commercial Items, applies to this solicitation. (xi) The clause at FAR 52.212-5, CONTRACT TERMS AND CONDITIONS REQUIRED TO IMPLEMENT STATUTES OR EXECUTIVE ORDERS COMMERCIAL ITTEMS (JUNE 2003) (DEVIATION), applies to this acquisition, which includes: FAR 52.219-8, Utilization of Small Business Concerns (OCT 2000). (xii) The clause at DFARS 252.212-7001(a), CONTRACT TERMS AND CONDITIONS REQUIRED TO IMPLEMENT STATUTES OR EXECUTIVE ORDERS APPLICABLE TO DEFENSE ACQUISITIONS OF COMMERCIAL ITEMS (OCT 2003), applies to this solicitation. Specifically the following clauses cited are applicable: DFARS 252.225-7001, Buy American Act and Balance of Payment Program; 252. 225-7016, Restriction on Acquisition of Ball and Roller Bearings, Alternate 1; DFARS 225-7036, Buy American Act, North American Free Trade Agreement Implementation Act, Balance of Payment Program, Alternate 1. DFARS 252.232-7003, Electronic Submission of Payment Requests. (xiii). The clauses at FAR 52.204-7, Central Contractor Registration (OCT 2003) and DFARS 252.204-7004, Alternative A (NOV 2003) apply to this acquisition. (xiv) All firms or individuals responding must be registered with the Central Contractor Registration (CCR). The Government reserves the right to award on a multiple award or on all or none basis. (xv). Local clause G-101, Wide Area Workflow, Receiving and Acceptance applies to this acquisition. In the near future, all vendors will be required to submit electronic invoices through the Wide Area Workflow, Receiving and Acceptance web site.
