Description

Subject: Request for MDACM ENTERPRISE EDITION INITIAL SITE LICENSE The software program, includes 3-years upgrade package, which includes 21 licenses each year. This is a required upgrade of the current MDA Case Management Software. The current version is not compatible with Office 2000. It is used by Occupational Medicine to manage our cases and generate meaningful information sharing. This software increases our ability to respond to the needs of our customers, employees, management, safety, injury compensation, and our private providers within the community. Data is entered at the time of service. Multiple users access the database to input patient or medical provider information. Task lists remind us to follow-up on cases. Queries list outlying cases for attention. Cases are compared to reasonably expected time-off for that diagnosis by national standards. Lost and restricted time is tracked. Reports are generated for work areas of the base and Occupational Medicine. This solicitation is sole sourced to the REED GROUP, Ltd., 4041 Hanover Ave., 2nd Floor, Boulder CO 80305 This is a combined synopsis/solicitation for commercial items prepared IAW the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being required and a written solicitation will not be issued. The following clauses are applicable to subject solicitation: FAR 52.212-1 (Instructions to Offerors), 52.212-2 (Evaluation - Commercial Items), 52.212-3 (Offeror Representations and Certifications - Commercial Items), 52.212-4 (Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items). IAW clause 52.212-2, the evaluation factors shall be: Price and Price related factors only. This solicitation incorporates provisions and clauses that are in effect through Federal Acquisition Circular (FAC)97-26. NAICS Code 238220. Place of performance/FOB Robins AFB, GA.
