Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This combined synopsis and solicitation is for the following commercial item(s): Monthly service for 23 (twenty-three) each cellular telephones. Qty: 12 months. This solicitation is iss ued as a request for proposal (RFP). Submit written offers (oral offers will not be accepted) via mail, email or fax. All Statement of Need and Evaluation Factor requirements must be met as they appear in this notice. This solicitation document and incorpo rated provisions and clauses are those in effect through Federal Acquisition Circular 97-14 and Defense Acquisition Circular 91-13. This procurement is being issued as 100% SMALL BUSINESS SET-ASIDE. Standard Industrial Code 4812 and NAICS Code 517212 apply to this procurement, with a small business size standard of 1,500 employees. Location of service to be performed: 5323 East McKinley Ave, Fresno, CA 93727. The provisions at FAR 52.212-1, Instructions to Offerors--Commercial Items, applies to this solici tation. The provisions at FAR 52.212-2, Evaluation-- Commercial Items, applies to this solicitation. The clause at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items, applies to this solicitation and the offeror must include a com pleted copy of this provision with their proposal. The clause at FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders--Commercial Items, applies to this solicitation. Specifically, the following clauses cited are a pplicable to this solicitation: FAR 52.222-26, Equal Opportunity; FAR 52.222-35, Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era; FAR 52.222-36, Affirmative Action for Handicapped Workers; FAR 52.222-37, Employment Reports on Disab led Veterans and Veterans of the Vietnam Era; FAR 52.222-42, Statement of Equivalent Rates for Federal Hires 52.222-48. The clause at DFARS 252.212-7001, Contract Terms and Conditions Required to Implement Statues or Executive Orders applicable to Defense Acquisition of Commercial Items, applies to this solicitation. Specifically, the following clauses, cited are applicable to this solicitation: DFARS 252.225- 7001, Buy American Act and Balance of Payment Program. The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this solicitation. The clause at FAR 52.224-1, Privacy Act Notification and FAR 52.224-2, Privacy Act, applies to this solicitation. The provision at FAR 52.215-1, Instructions to Offerors--Competitive Acquisitio ns applies to this solicitation. Award will be based on technical capability and past performance equal to lowest price, responsible offer. The Government intends to evaluate and award without discussions on this procurement. All proposals must be mailed t o Base Contracting Office, 144 MSG/LGC, 5323 E. McKinley Ave, Fresno, CA 93727, emailed to barbara.sentieri@cafres.ang.af.mil or faxed to (559) 454-5107. Proposals are required to be received no later than 2:00 PM PST, Thursday February 19, 2004. A per iod of acceptance must be indicated on the proposal and must be good for 30 days.
