Description

The GLAC (Great Lakes Acquisition Center) is soliciting quotations to provide Pharmacy Benefits Services for veteran beneficiaries for VISN 12 Community Based Outpatient Clinics beginning 4/12/04 for a period of one year from the date of the award in accordance with all specifications, work statement, terms and conditions contained therein (with four one-year options exercised at the government?s discretion).&nbsp&nbsp&nbsp&nbspThis service includes the first 10-day supply of emergent and controlled substance medications. All qualified offerors/firms who believe they can satisfy the solicitation requirements are invited and encouraged to provide their proposal. Offers, including options, shall be evaluated for fair and reasonable pricing, technical competency/capability to perform services and satisfactory past performance record. The Government anticipates award of a commercial item contract to the responsible offeror whose offer provides the best value to the Government. This is a Firm Fixed Price contract and this acquisition is a 100% small business set-aside. The size standard for North American Industry Classification System (NAICS) code 446110 is $6.0 Million. Offerors are reminded to complete all sections of the solicitation, including the Offeror Representations and Certifications-Commercial Items.&nbsp&nbsp&nbsp&nbsp&nbsp&nbspElectronic solicitation may be obtained FREE at the following URL: http://www.bos.oamm.va.gov/.&nbsp&nbsp&nbsp&nbspRequests for hard copy solicitation must be in writing to the Contracting Officer at the GLAC and accompanied by a check for $5.00 made out to the Department of Veterans Affairs.
