Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.(ii) This purchase request (PR)/solicitation will be issued as a request for quotation (RFQ)/request for proposal (RFP) under the following PR/solicitation number: F6SVFF40430100.(iii) This PR and the provisions and clauses incorporated are those in effect through Federal Acquisition Circular (FAC) 2001-05. (iv) This acquisition is set-aside 100% for small businesses/unrestricted in regards to size standards. The applicable NAICS is 611620, and the small business size standard is $6,000,000.(v) This RFQ/RFP contains one contract line item number (CLIN): Price will be submitted as follows: CLIN 0001, One day fitness training that links learning and physical fitness and conducting training at 8 Pacific Air Force (PACAF) Bases for their youth programs. Bases included Anderson AFB, Guam; Elmendorf AFB, AK; Eielson AFB, AK; Hickam AFB, HI; Kadena AFB, Japan; Misawa AB, Japan; Osan AB, Korea; Yokota AFB, Japan. Contractor is responsible for transportation, lodging and meals outlined in the attached Statement of Work (SOW).(vi) See the attached SOW for a full description of all requirements. (vii) The performance period will be from 1 May 2004 through 31 May 2004. The places of performance are outlined above in paragraph (v). Acceptance will be performed by the Government. The FOB Point is destination. (viii) The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. (ix) The contract will be awarded to the offeror that provides the best value to the government and who at a minimum meets the requirements of Paragraph 1.5 of the SOW. (x) All offers must include a completed copy of the provision at 52.212-3 , Offeror Representations and Certifications -- Commercial Items (See Attached)(use for acquisitions up to$25,000), (use Alt I for acquisitions over $25,000). (xi) The clause at 52.212-4 , Contract Terms and Conditions -- Commercial Items applies to this acquisition along with the following addenda: 52.219-1, Small Business Program Representations (Apr 2002). All offers must include a completed copy of this provision.
